Description of the medicine: Urapidil (Urapidil)

URAPIDIL *. 6 - [[3- [4- (ortho-methoxyphenyl) -1-piperazinyl] propyl] amino] -1, 3 dimethyluracil.

Produced in the form of hydrochloride.

Synonyms: Ebrantil, Ebrantil, Eupressyl.

The structure of the side chain has some similarities with cinnarizine.

Has hypotensive effect; Reduces peripheral vascular resistance. The effect is partially caused by a decrease in the flow of central sympathetic vasoconstrictor pulses, in part by peripheral a 1-adrenoceptor action. The mechanism of action is not fully understood.

In general, the drug is used to reduce blood pressure in hypertensive crises (see also Clofhelin), as well as in severe cases of hypertension, including cases that are resistant to other antihypertensive agents.

Assign intravenously and inside.

In acute and severe forms of hypertension administered intravenously to adults, usually at a dose of 0.025 g (25 mg), i.e. 1 ampoule (5 ml) of 0.5% solution (for 5 min). In case of insufficient effect, the same dose is administered after 2 minutes, and in case of insufficient effect of this dose, after 2 minutes a slow intravenous injection of 50 mg is administered. When the effect is achieved after the first, second or third injection, they pass to a slow drop infusion. For infusion 250 mg of urapidil is diluted in 500 ml of isotonic sodium chloride solution or 5-10% glucose solution. Enter with a perfusion pump at a rate of 9 - 30 mg per hour (an average of 15 mg / h).

The duration of administration depends on the severity of the disease, the effect obtained and the tolerability (no more than 7 days). After obtaining the desired effect, it is possible to switch to taking the drug inside.

For prolonged antihypertensive therapy, urapidil is administered internally in capsules. Begin usually with 30 mg 2 times a day or 60 mg 2 times a day, selecting a dose individually. The daily dose can vary from 30 to 180 mg. Take in the morning and evening while eating; Capsules are swallowed without chewing, washed down with a small amount of liquid.

When ingestion, urapidil is usually well tolerated, but dizziness, headache, general weakness, palpitations, gastrointestinal disorders, dry mouth; In addition, the phenomenon of orthostatic collapse. Sometimes there are skin allergic reactions, thrombocytopenia.

With rapid intravenous administration, a collapoid state can develop.

The drug is contraindicated in pregnancy. It is not assigned to children. Treatment of elderly patients begins with the appointment of smaller doses. Other antihypertensive drugs can enhance the action of urapidil. It is not recommended (in connection with the lack of sufficient experience) to use urapidil simultaneously with inhibitors of adenosine converting enzyme (see Captopril).

Do not prescribe urapidil (for outpatient treatment) to drivers of transport and to other occupations who require rapid physical and mental reactions.
