Sulpiride is a sulfonylbenzamide derivative. The same group includes tiaprid and some other psychotropic drugs. By its structure and some pharmacological properties, sulpiride is also close to metoclopramide (see).

Like metoclopramide has antiemetic effect; In addition, it has pharmacological properties characteristic of psychotropic drugs; Has a moderate antiserotonin and cataleptogenic effect, somewhat weakens the stimulating effects of phenamine. Sedation does not, does not enhance the effects of barbiturates and analgesics. Has no anticonvulsant activity.

Clinically, sulpiride is characterized as a drug with a "regulating" effect on the central nervous system, in which moderate neuroleptic activity is combined with some antidepressant and stimulating properties. Neuroleptic effect, perhaps due to the fact that the drug is an antagonist of dopamine receptors.

In psychiatric practice, sulpiride is used mainly in combination with other neuroleptics and antidepressants in conditions accompanied by lethargy, inhibition, anergy, including hallucinatory-delusional and affective (mainly repressive) disorders. As an activator is used for slow sluggish schizophrenia. Assign inside 100-1000 mg per day. In severe cases, starting with the administration of the drug intramuscularly at doses of 100 to 800 mg per day.

Sulpiride is also used for migraines, dizziness, and behavioral disorders in children. In these cases, adults are prescribed 100-300 mg per day; Children - 5 mg / kg per day.

In connection with the antiemetic and <cytoprotective> effect on the gastric mucosa (see Metoclopramide), as well as the "regulating" effect on the nervous system, sulpiride is used for peptic ulcer of the stomach and duodenum. The drug has no immediate antacid effect. As a rule, they apply against the background of therapy with a suitable diet. Usually prescribed inwards, starting with a dose of 200 mg per day (100 mg twice a day - in the morning and in the evening), increasing if necessary daily dose to 400 mg (200 mg twice a day).

The drug can be administered intramuscularly.

Sulpiride is usually well tolerated. However, pyramidal disorders, agitation, sleep disturbances, increased blood pressure, disturbances of the menstrual cycle can be observed; Cases of galactorrhea and gynecomastia are described.

The drug is contraindicated in patients with pheochromocytoma and with severe hypertension. Do not prescribe sulpiride to patients who are in a state of anxiety and psychomotor agitation.
