According to the chemical nature, cefotaxime is close to the cephalosporins of the first and second generations, but the structure features provide high activity against gram-negative bacteria, resistance to the action of β-lactamases produced by them. The drug has a wide spectrum of action; Acts bactericidal against gram-positive and gram-negative microorganisms resistant to other cephalosporins, penicillins and other antimicrobial agents.

Apply cefotaxime intramuscularly and intravenously; When ingested, it is not absorbed. With intramuscular injection, the drug is absorbed rapidly. The peak concentration in the blood plasma is observed 30 minutes after the injection. Bactericidal concentration in the blood is stored for more than 12 hours. The drug penetrates well into tissues and body fluids; Is found in effective concentrations in the pleural, peritoneal, synovial fluids. It is excreted in significant amounts with urine unchanged (about 30%) and in the form of active metabolites (about 20%). Partially excreted with bile.

Apply cefotaxime in infections caused by sensitive microorganisms: infections of the respiratory and urinary tract, kidneys, ear infections, throat, nose, septicemia, endocarditis, meningitis; Infections of bones and soft tissues, abdominal cavity, with gynecological infectious diseases, gonorrhea, etc.

For intramuscular injection, dissolve 0.5 g of cefotaxime in 2 ml (respectively 1 g and 2 g in 3 and 5 ml) of sterile water for injection. Enter deep into the gluteal muscle. For the introduction into a vein, dissolve 0.5 g in 2 ml (or 1 g in 4 ml and 2 g in 10 ml) of sterile water for injection. Enter slowly for 3 to 5 minutes. For drip (within 50 - 60 min), dissolve 2 g of the drug in 100 ml of isotonic sodium chloride solution or 5% glucose solution.

The usual dose of cefotaxime for adults and children over 12 years is 1 g every 12 hours. In severe cases, the dose is increased by 2 g every 12 hours or increases the number of administrations up to 3-4 times a day, bringing the total daily dose to the maximum - 12 g.

The usual dose for newborns and young children is 50-100 mg / kg per day, divided into separate receptions at intervals of 12 to 6 hours. For preterm infants, the daily dose should not exceed 50 mg / kg. In the case of impaired renal function, the dose is reduced. With the initial anuria (creatinine release 5 ml / min), usually reduce the dose by half.

With the use of cefotaxime, allergic reactions, digestive disorders, increased eosinophilia, leukopenia, neutropenia, increased liver function tests, alkaline phosphatase levels and nitrogen in the urine are possible. Symptoms of irritation at the injection site may develop, the temperature may rise.
