Description of the medicine: Troventolum (Troventolum)

Troventolum.

Tropine ester d, 1- (2-hydroxymethyl-2-phenyl) butyric acid iodomethylate.

Synonym: Truvent.

White or white with a slightly yellowish tinge crystalline powder. Slightly soluble in water.

By chemical structure is close to atropine, but, like an atrovent, is a quaternary ammonium derivative. In contrast to the atropine molecule, the trentene molecule contains an additional methyl group at the nitrogen atom of the tropane ring, as well as an ethyl group in the esterifying acid residue.

It is an anticholinergic substance that acts predominantly on m-holinoretseptory. In comparison with atropine, the effect of trententhol on the cholinergic receptors of the bronchial muscles is stronger and prolonged with less pronounced effect on the cholinergic receptors of other organs (heart, intestine, salivary glands). As a quaternary ammonium compound, trententhol does not penetrate the blood-brain barrier. The selective effect of troventol determines its advantages in comparison with atropine as a bronchodilator and its better tolerability.

Trenthol as a very active bronchodilator is used for chronic obstructive bronchitis, bronchiolospasm in chronic pneumonia, asthma attacks of mild and moderate severity, bronchiolospasm associated with colds (including in elderly patients).

Like the atrovent (ipratropium bromide), trententhol is most effective in bronchiolospasm caused by the hyperactivity of the cholinergic system, when adrenomimetics (see Orciprenaline) and methylxanthines (see Theophylline) are not effective enough.

There are indications of the possibility of using trventol for the diagnosis of latent bronchospastic syndrome.

Apply troventhal in the form of inhalations from aerosol cans. There are cylinders with two dosages: 12, 5 and 25 mg in a bottle. With a single pressure on the cylinder valve with 12, 5 mg, a single dose of roententhol 40 μg (0.04 mg) is released, and when the cylinder valve is pressed with 25 ml, 80 μg (0.08 mg) is released.

Prophylactic and therapeutic dose of the drug can vary in different patients from 40 to 160 mcg. The daily dose is 120 and 480 μg, respectively.

Begin with the appointment of 40 mcg, ie, one inspiration when you press the balllin valve with 12, 5 mg. In case of insufficient effect, two inhalations (two strokes) are made, ie, they increase the single dose to 80 mcg. Depending on the clinical effect and tolerability of the drug, a single dose is increased to 80-160 μg, while for convenience use a balloon with 25 mg atrovent (1 - 2 clicks). With sufficient effect from a single dose of 40 mcg continue to use a balloon with 12.5 mg of trentol.

Aerosol inhalations are repeated every 4 to 6 hours.
