Congenital clubfoot

Congenital clubfoot is manifested by the persistent contraction of the foot. This is one of the most common malformations of the musculoskeletal system. Deformation is more common in boys, it is one- and two-sided. In this case, there is a shortening of the tendons and muscles of the foot, a shift in the attachment points of the Achilles tendon to the inside of the calcaneus, the underdevelopment of the ligamentous and articular apparatus of the ankle joint.

From birth, the child is found to be in the wrong position of the foot. It has a number of deformations: an equnus-plantar flexion of the foot, supination, raising the inner part of the foot and lowering the outer one, adduction - the front section is shown and the longitudinal arch of the foot is enlarged (so-called hollow foot). Along with the perverse position of the foot, there is atrophy of the calf muscles, especially the gastrocnemius muscle. In the ankle joint, only rocking movements are possible. Often there is a shortening of the shin, progressing with the growth of the child.

Differentiation of clubfoot is necessary from other congenital deformities of the foot: arthrogryptic, amniotic and paralytic.

Treatment of clubfoot can be operative and conservative. Conservative treatment is carried out in the first year of life. To correct the deformation begin with a 2-nedepinnogo age by gradual gypsum. At each stage, when the gypsum bandage is changed, the position of the foot is gradually corrected. Plaster casts are applied from the fingertips to the knee joint, with a sharp deformation, to the upper third of the thigh. Change dressings produced once in 10-12 days. If the ineffectiveness of conservative treatment, as well as late treatment, use operative treatment, which is carried out at 6-12 months of age. The complex of rehabilitation measures necessarily includes physiotherapeutic procedures (muscle electrical stimulation, thermal procedures, ultrasound) and exercise therapy for developing movements in the ankle joint.

The prognosis for restoring the functional state of the limb is usually favorable.
