Abscess of the Douglas space

The abscess of the Douglas space is mainly due to perforated appendicitis, perforation of the diverticulum of the colon, less often it is a residual abscess in the treatment of diffuse forms of peritonitis. In connection with the fact that the abscess has no direct contact with the anterior abdominal wall, when palpation of the abdominal wall of pathological signs and symptoms, it is practically impossible to detect. Patients complain of a feeling of heaviness, raspiranie, pain in the lower half of the abdomen, frequent and painful urination, frequent stools or diarrhea with tenesmus. With digital rectal or vaginal examination on the anterior wall of the rectum, a painful infiltration with a softening in the center is determined. A differential diagnosis with inflammatory diseases of the female sexual sphere is needed.

Treatment. Transrectal or transvaginal opening and drainage of the abscess.

The prognosis for a single pelvic abscess is usually favorable.
