Perforation of the appendage

Perforation of the appendage with the development of diffuse (diffuse) or diffuse peritonitis. Against the background of a characteristic clinical picture of acute appendicitis, pain suddenly sharply increases (the moment of perforation), and then spreads throughout the abdomen. The general condition of the patient deteriorates rapidly. Increases intoxication, gradually progressing paresis of the gastrointestinal tract, manifested by bloating, regurgitation and vomiting with stagnant gastric contents. Other symptoms are dry, lax tongue, tachycardia that does not correspond to the temperature level, and high leukocytosis in the blood. In physical examination, spilled muscle tension and soreness over the entire abdomen with positive symptoms of irritation of the peritoneum, complete absence of peristaltic intestinal noise are determined.

Treatment is an operation. However, with severe intoxication, clear signs of diffuse peritonitis, preoperative preparation is required, which is performed by an anesthesiologist for 2-3 hours together. Surgical intervention consists in appendectomy, washing the abdominal cavity, draining it. When diffuse peritonitis after the operation, peritoneal lavage or repeated relaparotomy (initially every other day) is performed with the abdominal cavity washed.

The prognosis is serious.
