Parotitis epidemic (mumps)

Viral disease with general intoxication, an increase in one or more salivary glands, often a lesion of other glandular organs and the nervous system. Pathogen - a virus of spherical form, which has a tropism to the glandular and nervous tissues. Low resistance to physical and chemical factors. The source of the disease is a sick person. Infection occurs by a drip, the possibility of a contact transmission path is not ruled out. The virus is found in the saliva at the end of the incubation period for 3-8 days, after which the virus is stopped. Outbreaks are often local in nature.
Symptoms and course:
The incubation period is usually 15-19 days. There is a short prodromal (initial) period, when weakness, malaise, pain in the muscles, headache, cognition, sleep, appetite are noted. With the development of inflammatory changes in the salivary gland there are signs of her defeat (dry mouth, ear pain, aggravated by chewing, talking). The disease can occur in both mild and severe forms.
Depending on this temperature can be from low-grade figures to 40 C, intoxication also depends on the degree of severity. A characteristic manifestation of the disease is the defeat of the salivary glands, most often parotid. The gland is enlarged, pain appears on the palpation, which is especially pronounced in front of the ear, behind the ear lobe and in the region of the mastoid process. Murson's symptom-inflammatory reaction in the region of the excretory duct of the affected parotid gland is of great diagnostic significance. The skin above the inflamed gland is tense, glossy, the swelling can spread to the neck. The enlargement of the gland lasts usually 3 days, the maximum swelling lasts 2-3 days. Against this background, various, sometimes severe complications can develop: meningitis, meningoencephalitis, orchitis, pancreatitis, labyrinthitis, arthritis, glomerulonephritis.
Treatment:
Bed rest for 10 days. Compliance of milk and vegetable diet, restriction of white bread, fat, coarse fiber (cabbage).
With orchitis appointed Suspension, prednisolone 5-7 days but the scheme. With meningitis, corticosteroids are used, a lum-
Ballpoint punctures, intravenously administered 40% solution of Urotropin. With the development of acute pancreatitis, a thin, sparing diet is prescribed, atropine, papaverine, cold on the stomach, with vomiting - aminazine and drugs inhibiting enzymes - gordoks, counter trasil.
The prognosis is favorable.
Prevention:
In children's institutions, in case of detection of cases of mumps, quarantine is established on day 21, active medical supervision. Children who have had contact with sick mumps are not admitted to children's institutions from the 9th day of the incubation period to 21, they are given placental gamma globulin. Disinfection in the outbreaks is not carried out.
