POLYMIXINE M SULFATE (Polymyxini M sulfas).

Polymyxin is a group of related antibiotics produced by spore-forming soil bacteria Bacillus polymyxa or other related microorganisms. According to their chemical structure, polymyxins are complex compounds, including residues of polypeptides. Different polymyxins have an additional letter designation. Polymyxin M is a type of polymyxin.

Polymyxin M sulfate is white or white with a creamy shade of loose powder or a porous mass, sweetish-bitter in taste. Easily soluble in water.

The activity of the drug is determined by biological means and is expressed in units of action (ED); In 1 mg contains 8000 units.

Polymyxin M acts predominantly on gram-negative microorganisms: it inhibits the growth of intestinal and dysenteric rods, typhoid and paratyphoid sticks; Is effective against Pseudomonas aeruginosa. It does not affect Proteus, gram-positive cocci, mycobacteria, fungi.

Polymyxin M is low toxicity when applied topically. When ingested, it is poorly absorbed from the gastrointestinal tract and does not have a toxic effect on the body. When parenteral administration of the drug is toxic: it has nephro- and neurotoxic effects.

Locally, polymyxin M is used in various sluggish processes: sluggish healing wounds, infected burns, necrotic ulcers, pressure sores, purulent otitis, inflammatory diseases of the eyes and ear, abscesses and other purulent diseases caused by Pseudomonas aeruginosa and Gram-negative microorganisms. The drug is used as a liniment or solution, which is prepared immediately before use at a rate of 10 000 - 20 000 units per 1 ml of isotonic sodium chloride solution or 0.5-1% solution of novocaine.

Solutions are used for wetting tampons, dressings, for irrigation and in the form of drops.

Liniment is applied a thin layer on the affected surface (after removal of pus and necrotic masses) 1 to 2 times a day, and for burns 2 to 3 times a week. The daily dose for topical application is no more than 20 000 units (2 mg) per 1 kg of body weight. Duration of treatment is 7-10 days.

Inside appoint polymyxin M in gastrointestinal diseases (colitis, enterocolitis, gastroenterocolitis), caused by gram-negative bacteria and Pseudomonas aeruginosa. It is recommended to use the drug for acute and chronic dysentery in cases when other antibiotics are ineffective. The drug can be used to prepare patients for operations on the gastrointestinal tract.

Polymyxin M can be administered in combination with other antibiotics acting on Gram-positive microorganisms.
