Description of the medicine: Pyrazinamide (Pirazinamidum)

PYRASINAMIDE (Pirazinamidum).

Pyrazinecarboxylic acid amide.

Synonyms: Tizamid, Aldinamid, Cavizide, Eprazin, Farmizina, Isopyrasin, Novamid, Piraldina, Pyracinamide, Pyrazinamide, Tebrazid, Tisamid, Zinamide.

White crystalline powder. Soluble in water when heated.

More active on tuberculostatic activity than PASC, although inferior to isoniazid, streptomycin, rifampicin, cycloserine, ethionamide, kanamycin, florimycin. Effects on mycobacteria resistant to other pro-TB drugs I and II series. The drug penetrates well into the centers of tuberculous lesion. Its activity does not decrease in the acidic environment of caseous masses, and therefore, it is often prescribed for caseous lymphadenitis, tuberculomas and caseous-pneumonic processes.

In the treatment with pyrazinamide alone, the rapid development of resistance to mycobacteria of tuberculosis is possible, therefore, it is usually combined with other anti-tuberculosis drugs (isoniazid, streptomycin, etc.).

The drug is especially effective in patients with newly diagnosed destructive tuberculosis.

The daily dose of pyrazinamide for adults is 1, 5 - 2, 0 g, with good tolerability of up to 2.5 grams per day. Take inside after eating 1 g 2 times (rarely 0.5 g 3 - 4 times) per day. The daily dose for patients over 60 years old and with a body weight of up to 50 kg is 1, 5 g. Children are prescribed at the rate of 0.02-0.03 g (20-30 mg) per 1 kg of body weight per day (daily dose is no more than 1, 5 g).

In the treatment with pyrazinamide, allergic reactions may occur: dermatitis, eosinophilia, febrile reactions, etc. Dyspeptic phenomena, worsening of appetite, headache, and occasionally - increased excitability, anxiety are also possible. With prolonged use may have toxic effects on the liver.

In the process of treatment with pyrazinamide, it is necessary to follow the liver function, conducting biochemical tests (timol assay, determining the level of bilirubin, the study of serum glutaminic acid serum aminofurase, etc.). If there is a change in liver function, stop taking the drug. To reduce the toxic effect of pyrazinamide, the use of methionine, lipocaine, glucose, vitamin B is recommended.

There are data on the delay in the body under the influence of pyrazinamide uric acid and the possibility of increasing the arthritic pain in the joints, it is therefore advisable to determine the content of uric acid in the blood.

Contraindications: a dysfunction of the liver and gout.
