NADROPARIN CALCIUM (Nadroparin calcium).

Synonym: Fraxiparin, Fraxiparin.

It is the calcium salt of depolymerized heparin, obtained from the intestinal mucosa of pigs. The average molecular weight is 4000-5000 daltons.

Has a direct anticoagulant effect associated with direct inhibition of coagulation factors Xa and IIa, as well as an increase in the blocking effect of antithrombin III on factor Xa.

Has anti-inflammatory and immunosuppressive properties, somewhat reduces the level of cholesterol and β-linoproteins in the blood serum.

When administered subcutaneously, the bioavailability is 98%, C max is 4-6 h, T S is 3.5 h (with repeated administration 8-10 h).

It shows a rapid and rather long (in comparison with the usual heparin) antithrombotic effect with a relatively lower anticoagulant activity.

Apply nadroparin calcium (as well as other low molecular weight heparins) in orthopedic and surgical practice for the prevention of thromboembolic complications, for the treatment (resorption) of deep venous thrombosis, for thromboembolism of pulmonary arteries, acute coronary syndrome and other pathological conditions accompanied by thrombosis. They are also used to prevent hypercoagulation in the extracorporeal circulation system during hemodialysis.

Enter usually subcutaneously (in the cellulose of the stomach) 1-2 times a day, sometimes intravenously (under the control of blood coagulation indicators).

For prophylaxis of deep vein thrombosis and pulmonary artery thromboembolism during surgical interventions, subcutaneous injection of 0.3 ml (2850 IU) for 2-4 h before the operation, then 1 time per day for 7 days; At orthopedic operations - depending on the body weight of 0.2-0.4 ml (38 IU / kg) 1 time a day before and within 3 days after the operation, then 0.3-0.6 ml (57 IU / Kg) for at least 7 days.

With unstable angina and acute myocardial infarction without a Q wave, intravenously administered bolus at the rate of 86 IU / kg, then subcutaneously the same dose 2 times a day for 4-8 days.

When the drug is used, hemorrhage and thrombocytopenia, allergic reactions (local and general), rarely - hematomas at the injection site are possible.

Nadroparin calcium is contraindicated in thrombocytopenia, a tendency to bleeding, including ulcers of the stomach, with violations of cerebral circulation, during pregnancy. During the treatment it is necessary to determine the number of platelets.

An antagonist with an overdose of the drug (increased bleeding) is protamine sulphate (see).

You can not use Fraxiparin simultaneously with antiaggregants (including acetylsalicylic acid), as well as with NSAIDs and anticoagulants.
