Description of the medicine: Metoclopramide (Metoclopramidum)

METOKLOPRAMID (Metoclopramidum). 4-Amino-5-chloro-N- (2-diethylamino-butyl) -2-methoxybenzamide hydrochloride.

By chemical structure is close to sulpiride (see) and dimetramid (see).

Metoclopramide is a specific blocker of dopamine (D 2) receptors, as well as serotonin (5-HTZ) receptors.

The drug has an antiemetic effect, soothes hiccups and, in addition, has a regulating effect on the functions of the gastrointestinal tract. The tonus and motor activity of the digestive organs are strengthened. The secretion of the stomach does not change. There are indications that the drug contributes to the healing of stomach and duodenal ulcers.

Metoclopramide is used as an antiemetic for nausea, vomiting associated with anesthesia, radiation therapy, side effects of drugs (digitalis preparations, cytostatics, antibiotics, etc.), diet disorders, etc.

On vomiting of vestibular genesis it does not work.

In gastroenterological practice, the drug is also used in the complex treatment of peptic ulcer of the stomach and duodenum, gastritis, dyskinesia of the abdominal cavity, postoperative intestinal paresis, with flatulence and other diseases.

The therapeutic effect is associated with an increase in the tone of the stomach and intestines, acceleration of emptying of the stomach and pylorus, a decrease in hyperacid stasis.

There are data on the effectiveness of metoclopramide in the treatment of dyspepsia (repeated vomiting, nausea) in severe cardiac patients (myocardial infarction, heart failure) and with vomiting of pregnant women.

The drug also found application as a tool that facilitates and improves X-ray diagnosis of diseases of the stomach and small intestine.

There are data on the high efficacy of metoclopramide in migraine and the successful use of the drug in Tourette's syndrome (generalized tics and vocalization in children).

Apply metoclopramide inside, and in severe cases parenterally.

Inside, adults are usually given 10 mg (1 tablet) 3 times a day (before meals). Intramuscularly (or intravenously) 1 ampoule (2 ml = 10 mg of the drug) 1 to 3 times a day is administered. Children are prescribed in smaller doses according to their age (children over 6 years of age - 1/2 - 1 tablet 3 times a day).

Tablets are swallowed, not liquid, squeezed with a small amount of water.

For the X-ray study, 1 to 2 ampules (10-20 mg) are administered to an adult intravenously or given inside (15-15 minutes before the start of the study) 15-30 mg.

The drug is usually well tolerated. In rare cases, extrapyramidal disorders characteristic of parkinsonism (hyperkyneto-dystonic phenomena) are possible. In the mechanism of these side effects, the antagonistic effect of metoclopramide on the dopamine receptors of the brain seems to play a role. To remove these phenomena, caffeine is administered parenterally.
