Carbachol (Carbacholinum).

N- (~) -Karbamoiloksietil) -trimetilammoniya chloride.

The white crystalline powder with a faint odor of aliphatic amines. Hygroscopic. It is very easily soluble in water, it is difficult - in alcohol, insoluble in ether and chloroform. The solutions (pH 5, 0 - 7, 0) + sterilized at 100 C for 30 min.

On chemical structure and pharmacological properties similar to acetylcholine; is more active and has a longer action, as not hydrolyzed cholinesterase.

Drug resistance allows to use it not only for parenteral administration, and for oral administration. For oral and parenteral administration, the drug is rapidly absorbed.

Carbachol zffektivno (more than acetylcholine) increases the tone of the muscles of the bladder and bowel. For topical application (eye drops) lowers intraocular pressure in glaucoma.

The dose for adults ingestion 0.0005 - 0.001 g (0.5 - 1 mg) subcutaneously and intramuscularly 0.0001 - 0.00025 g (0.1 - 0.25 mg), 2 - 3 times a day. When administered intravenously (0.00005 g = 0.05 mg) should be very careful: the drug is administered very slowly. The doses due to the high activity of the drug should not be exceeded.

Higher doses for adults: single - into 0.001 g, 0.0005 g under the skin; daily - into 0.003 g, 0.001 g under the skin

In glaucoma carbachol solution (0.5 - 1%) was instilled into the conjunctival sac 2 - 6 times per day. The concentration of carbachol instillation and frequency should be individualized for each patient, as due to the strong contraction of the pupil and the ciliary muscle spasm possible headache and pain in the eyes.

Contra-indications, possible complications and assistance measures are the same as for acetylcholine. When receiving carbachol sometimes appear sensation of heat, excessive salivation, nausea, bradycardia; These phenomena are in dose reduction.
