TROPAFEN (Tropaphenum).

Tropine ester of a-phenyl-b- (p-acetoxyphenyl) -propionic acid hydrochloride.

Synonym: Tropodifene hydrochloride.

White or white with a weak, greyish-creamy hue, crystalline powder. Easily soluble in water and alcohol: pH 2% aqueous solution 5, 0 6, 5.

It belongs to the group of a-adrenoblockers. It causes a decrease or complete removal of a-adrenergic effects caused by adrenaline, norepinephrine and adrenomimetic substances, as well as irritation of sympathetic nerve fibers. Strongly dilates the peripheral vessels and causes a drop in blood pressure. It has weak anticholinergic properties.

Applied for the treatment of diseases associated with impaired peripheral circulation (endarteritis, Raynaud's disease, acrocyanosis, etc.). Reducing or completely removing the spasms of blood vessels, the drug helps to increase blood circulation, reduce pain and improve the functional state of the limbs (facilitating walking). The property of tropaphene to improve peripheral circulation gives grounds to apply it also in the treatment of trophic ulcers of limbs and sluggish healing wounds.

The drug is used to stop hypertensive crises, as well as to prevent hypertensive reactions caused by increased blood levels of catecholamines during anesthesia and surgical interventions.

There is evidence of the ability of tropaphene to reduce platelet hyperaggregation and reduce the viscosity of arterial and venous blood. "

Administer tropafen under the skin, intramuscularly or intravenously.

With endarteritis and other diseases accompanied by impaired peripheral circulation, 0.5-1.2 ml of 1% or 2% solution 1-2-3 times a day is injected subcutaneously or intramuscularly. The course of treatment is 10 - 20 days or more. If necessary, repeat treatment.

To stop and prevent hypertensive crises, inject 0.5-1 ml of 1% or 2% solution subcutaneously or intramuscularly.

In connection with the antiadrenergic effect, tropaphene is a valuable tool for the diagnosis and treatment of catecholamine-producing adrenal tumors (pheochromocytoma, pheochromoblastoma).

For pharmacological diagnosis of pheochromocytoma and pheochromoblastoma, a combined sample with histamine and tropaphene is performed. Hypertensive reaction to histamine and hypotensive - to the introduction of tropaphene gives in these cases objective information. After the administration of histamine, systolic and diastolic arterial pressure in patients with such tumors increases by 60 and 40 mm Hg. Art.
