Cromolin-sodium is a specific agent used in asthma and bronchospastic conditions. The basis of its action is the ability to inhibit the degranulation of mast cells (mast cells) of the mucous membrane of the respiratory tract and delay the release of mediator substances from them that contribute to the development of bronchiolospasm, allergies and inflammation (bradykinin, "slow-reacting substance", histamine, etc.). It is possible that the effect of the drug is also partly related to the blockade of receptors specific for inflammatory mediators, as well as to cholino- and adreno-mediators of lymphoid cells. The properties of conventional receptor blockers, kromolin sodium does not possess.
The action of cromolyn-sodium is clearly manifested when applied topically in the form of inhalation. When ingested, the effect does not develop.
Cromolin sodium is effective in patients with bronchial asthma and has a warning effect when applied prior to the development of an attack of bronchial asthma. Continuous long-term use of the drug reduces and relieves bronchial asthma attacks, reduces the need for bronchodilators and corticosteroids. For the relief of acute attacks of the drug is not used. Cromolin sodium is most effective in patients of relatively young age who have not yet developed chronic changes in the lungs. The effect of the drug is most pronounced in atopic form of bronchial asthma, but the effect is also observed in the case of an infectious-allergic form of the disease, in asthmatic bronchitis, pneumosclerosis with asthma attacks, especially in the presence of an allergic component.
The drug is used in the form of powder in capsules for inhalation. Each capsule contains 0.02 g (20 mg) of cromolyn-sodium. Spraying of the powder and its inhalation are carried out using a special pocket turbo-inhaler ("Spingaler"), in which a capsule with the preparation is placed. When you press the top (lid) of the inhaler, the capsule is punctured and the powder enters the respiratory tract when inhaled. To empty the capsule requires 4 energetic breaths.
Inhalation produced daily. Begin with the use of 1 capsule 4 times a day at intervals of 4 to 6 hours. In severe cases, the dose is sometimes increased to 8 capsules per day (every 3 hours, 1 capsule). After improvement, the number of inhalations is gradually reduced, selecting an effective maintenance dose.
Cromolin sodium can be used in combination with bronchodilators and corticosteroids.
