Description of the medicine: Pyromecaine (Pyromecainum)

Pyromecainum. 2, 4, 6-Trimethylanilide-1-butyl-pyrrolidinecarboxylic acid-2-hydrochloride, or 1-butyl-2,3,4-trimethyl-2-pyrrolidinecarboxanilide.

Synonyms: Bumecain hydrochlorid, Bumecaini hydrochloridum, Pirromecain.

White or white with a weak cream shade of crystalline powder. Soluble in water, easily - in alcohol. Solutions (pH 2% solution 4, 4 - 5, 4) are prepared on isotonic sodium chloride solution, sterilized at +100 ° C for 30 minutes.

Local anesthetic. The chemical structure has a similarity with trimecaine. It also has antiarrhythmic effect.

As an anesthetic is used for surface anesthesia.

In ophthalmic practice, a 0.5% solution of 1 to 2 drops is used; In otorhinolaryngological practice - 1 - 5 ml of 1 - 2% solution (if necessary add 0.1% solution of epinephrine hydrochloride 1 drop for every 2 - 3 ml of pyromecaine); For anesthesia during endoscopic examinations - 2 - 5 ml of 2% solution; In stomatology - 1 - 5 ml of 1 - 2% solution or 5% of pyromecaine ointment.

As an antiarrhythmic drug, pyromecaine is prescribed for ventricular arrhythmias: with myocardial infarction, cardiac glycoside intoxication, etc. Introduce pyrocomaine intravenously as a 1% solution in a 5% solution of glucose slowly at a rate of 1 ml / min at a dose of 50-100 mg (5 - 10 ml of a 1% solution) for 3 to 5 minutes. For long-term therapy, pyromecaine is injected intravenously slowly at a rate of 1 ml / min at a dose of 100 mg (10 ml solution) 2 to 3 times with an interval between administrations of 15-20 minutes. The achieved antiarrhythmic effect is maintained by the drip administration of the drug at a rate of 2 to 3 mg / min (8 to 12 drops per minute). In acute myocardial infarction, pyromecaine is administered: 1) at a dose of 100-150 mg (10-15 ml of a 1% solution) intravenously slowly for 3 to 5 minutes (single bolus); 2) at a dose of 100 mg (10 ml of a 1% solution) 2 times with an interval between administrations of 15-20 min with a drip introduction - a two-time bolus against infusion.

The duration of the infusion depends on the nature and course of the process and the effectiveness of the therapy.

The average single dose for intravenous administration of a 1% solution of 0.1 g (100 mg).

The highest daily dose when administered to a vein is 1.5 g (1500 mg).

With the use of pyromecaine, general weakness, nausea, vomiting, dizziness, a drop in blood pressure, a collapoid state are possible. If necessary, vasoconstrictors are recommended. Intravenous administration should be carried out only in a hospital setting.

Forms of issue: 0,5%; 1% and 2% solutions in 10 ml ampoules, 1% solution in 5% glucose solution for intravenous administration (Sol.Ro.mecaini 1% cum glucoso pro injectionibus) in 10 ml ampoules; 5% pyromecaine ointment.

Storage: List B.

Ointment pyromecaine 5% (Unguentum Pyromecaini 5%). Used as an anesthetic for inflammatory diseases of the oral mucosa. Apply a thin layer on the painful area once or twice for 2 to 5 minutes. The dose of ointment should not exceed 1 g.
