Description of the medicine: Celanidum (Celanidum)
The primary (genuinic) glycoside is digilanide, or lanatoside C, from the leaves of digitalis woolly.
Synonyms: Isolanide, Lanatoside C, Cedilanid, Cedisanol, Cedistabil,
Ceglunat, Celadigal, Cristalanat C, Digilanid C, Isolanid, Lanacroist,
Lanatigen C, Lanatosidum C.
Colorless or white crystalline powder; odorless. It is very slightly soluble in water, it is difficult in alcohol.
It acts on the heart like other digitalis glycosides, however, it has a quick effect and has a relatively small ability to cumulate. Active when taken orally; when introduced into a vein, it acts at almost the same rate as strophanthin. Compared with strophanthin, it has a stronger bradycardic effect.
The biological activity of celanide is 14000 - 16000 ICE or 3200 3800 KED in 1 g.
Used for acute and chronic circulatory failure
II and III degree, tachysystolic form of atrial fibrillation, supraven tricular form of paroxysmal tachycardia.
It is prescribed orally in tablets (0.25 mg each) or in drops (0.05% solution), as well as intravenously (0.02% solution for injection).
To obtain a quick effect, 0.2 - 0.4 mg (1 - 2 ml of a 0.02% solution) is administered intravenously 1 to 2 times a day. Inside, take, starting with 0.25 - 0.5 mg (1 - 2 tablets) or 10 - 25 drops of a 0.05% solution 3-4 times a day (daily doses sometimes exceed the higher daily doses prescribed by GPC). Upon achievement of the therapeutic effect (with intravenous administration, usually on the 2nd - 3rd day, and when taken orally on the 3rd - 5th day), the daily dose is reduced to the maintenance dose: 0.4 - 0.2 mg (2-1 ml 0 , 02% solution) intravenously, 0.5 - 0.25 mg (2 - 1 tablet) or 40 - 20 - 10 drops of a 0.05% solution inside. For long-term maintenance therapy, 1/2 tablet is prescribed orally 2 times a day.
Higher doses for adults inside: single 0.0005 g (0.5 mg), daily 0.001 g (1 mg); into a vein: single 0.0004 g (0.4 mg), daily 0.0008 g (0.8 mg).
Possible complications and contraindications are the same as for other digitalis glycosides.
Method of release: tablets of 0,00025 g (0.25 mg); 0.05% solution in 10 ml vials (0.5 mg in 1 ml) for oral administration and 0.02% solution in 1 ml ampoules (0.2 mg in 1 ml).
