Description of the medicine: Milrinone (Milrinone)

MILRINON (Milrinone) *. 1, 6-Dihydro-2-methyl-6-oxo [3, 4'-bipyridine] -5-carbonitrile.

Produced in the form of lactate.

Synonym: Primacor.

By structure and action is close to amrinone. Instead of an amino group, it contains a CN group and has an additional methyl radical. More active than amrinone, and better tolerated.

It is intended (like amrinone) only for short-term therapy of acute heart failure. Enter intravenously first ("loading dose") at a rate of 50 μg / kg (0.05 mg / kg) for 10 minutes (about 0.5 μg / kg per minute). The maintenance dose is 0.375-0.75 μg / kg per minute up to a total dose of 1, 13 mg / kg per day.

Duration of administration depending on the effect 48 - 72 h.

Possible side effects: hypotension, pain in the heart, hypokaemia, arrhythmias.

Contraindicated after acute myocardial infarction, during pregnancy.

Treatment should be performed under careful hemodynamic control. In case of impaired renal function, they are prescribed in reduced doses. When hypokalemia should be given drugs of potassium.

Form release: 0.1% solution in ampoules of 10 ml (10 mg).

The oral form of the preparation is being developed.
