Description of the medicine: Etmozin (Aetmozinum)

ETMOSINE (Aetmozinum). 2-Carbethoxyamino-10- (3-morpholinopropionyl) -phenothiazine hydrochloride 7

Synonyms: Ethmosine, Etmozin, Moracizin hydrochlorid.

White or white with cream-colored crystalline powder. It is soluble in water, it is difficult - in alcohol. It's getting dark in the light. Aqueous solutions for injection (pH 2, 5% solution 3, 3 -4, 2) are prepared by filtering through a sterilizing filter.

The chemical structure is similar to that of aminazine and other phenothiazine derivatives. However, neuroleptic activity of ethmosin does not possess, but is an effective antiarrhythmic agent.

An important feature of the chemical structure of ethmosin, which distinguishes it from amine nasin and its analogs, is the presence in the side chain of the ethmosine nitrogen of the propionyl group (CO-CH 2 -CH 2 -) in place of the propyl (CH 2 -CH 2 -CH 2 -) y Aminazine. In this respect, ethmosin is close to the anti-anginal drug "Nonahlazin" (see).

Etmozin refers to antiarrhythmic drugs of the I group (quinidino of similar action). It also has a moderate coronary, spasmolytic, m-cholinolytic effect.

Applied with extrasystoles, paroxysmal tachycardia and paroxysmal atrial fibrillation, occurring in IHD, as well as arrhythmias of another etiology. Effective in arrhythmias caused by an overdose of cardiac glycosides. Comparative studies show that ethmosine is a very effective drug for ventricular arrhythmias.

Assign inside, intramuscularly or intravenously.

With extrasystole taken internally, starting with a dose of 75 - 200 mg per day (25 - 50 mg 3 - 4 times a day). Atrial arrhythmias buy the drug in smaller doses than the ventricular. If the effect does not develop, the dose is increased to 300-400 mg per day, and with stable arrhythmias - up to 600 mg per day (100 mg 6 times a day). (Ventricular arrhythmias use up to 800 mg of ethmosin per day.). When the therapeutic effect is achieved, the drug continues to be administered in maintenance doses (the usual decrease by 1/3).

The duration of treatment depends on the form of arrhythmia and the effectiveness of therapy and is usually 7 to 45 days.

The intake of ethmosin inside can be combined with the introduction of it into a vein or intramuscularly.

For arresting paroxysmal tachycardia attacks, the drug is administered intravenously or intramuscularly.

Single dose for intravenous or intramuscular injection is 50 - 100 - 150 mg (2 - 4 - 6 ml 2, 5% solution). The daily dose should not exceed 600 mg.

For intramuscular injection, dilute 2 - 4 ml of a 2, 6% solution of ethmosin in 1-2 ml of 0.25-0.5% solution of novocaine. For introduction into the vein, dilute 2 ml of a 2, 5% solution of ethmosin (50 mg) in 10 ml of isotonic sodium chloride solution or 5% glucose solution. Enter slowly - within 5 - 7 minutes.

With good tolerance, it is possible to increase the dose of ethmosin up to 4-6 ml of a 2, 5% solution (100-150 mg). If necessary (insufficient effect, renewal of arrhythmia) is administered additionally up to 3 times a day at intervals of 1, 5 - 2 hours.
