AYMALIN (Ajmalinum).

The alkaloid contained in some species of Rauwolfia (Rauwolfia serpentina Ven., Etc.) (See Reserpine.).

By chemical structure belongs to the group of indole derivatives.

Synonyms: Ajmaline, Arythmal, Cardiorythmine, Gilurytmal, Ritmos, Tachmalin and others.

White or white with a slightly yellowish tinge crystalline powder. Very little soluble in water, it is difficult - in alcohol. The hydrochloride and the aymaline acetate are readily soluble in water.

Aymalin (unlike reserpine, also contained in rauwolfia) does not have neuroleptic activity, moderately reduces blood pressure, strengthens coronary blood flow a little, has a negative inotropic effect and a moderate adrenolytic effect.

A characteristic feature of Aymalin is antiarrhythmic properties. It lowers the excitability of the myocardium, lengthens the refractory period, inhibits atrioventricular and intragastric conduction, somewhat depresses the automatism of the sinus node, suppresses impulse formation in ectopic foci of automatism. Refers to antiarrhythmic drugs of the I group.

Aymalin is able to stop attacks of atrial fibrillation and paroxysmal tachycardia.

It is also effective in disorders of rhythm associated with intoxication with digitalis preparations.

Assign intravenously, intramuscularly and orally; More effective with parenteral administration.

To stop acute attacks of atrial fibrillation and paroxysmal tachycardia, 0.05 g (2 ml of a 2, 5% solution) in 10 ml of isotonic sodium chloride solution or 5% glucose solution is administered intravenously. Enter slowly - within 7 - 10 minutes. If necessary, during the day, you can reintroduce another 0.05 g. You can inject the drug drip: 2 - 4 ml 2, 5% solution in 100 - 200 ml isotonic sodium chloride solution. Intramuscularly inject 0,05 g (2 ml of 2, 5% solution) every 8 hours to 0.15 g per day (ie up to 3 times). If necessary, the drug is administered for several days in a row.

When the therapeutic effect is achieved, from the parenteral administration, 1 to 2 tablets (0.05 to 0.1 g) are taken orally 3 to 4 times a day (within 1 to 3 weeks).

Aymalin is usually well tolerated. Individual patients have hypotension, general weakness, nausea, vomiting; With intravenous administration - a feeling of heat.

Contraindications (especially for intravenous administration): severe damage to the conduction system of the heart, severe sclerotic and inflammatory changes in the myocardium, circulatory failure of grade III, severe hypotension.

Caution is required when administering Aimalin to patients with a recent myocardial infarction: a decrease in blood pressure and conduction disorders are possible.

Forms of release: tablets of 0.05 g; 2, 5% solution in ampoules of 2 ml (25 mg in 1 ml).
