ALLAPININ (Allapininum).

Lappakonitin alkaloid hydrobromide (with some admixture of other concomitant alkaloids), obtained from the grass of aconite (fighter) of the white-fronted Asonitum leucostomum Worosch., Family. Ranunculaceae.

White or white with a slightly creamy hue of crystalline powder. Slightly soluble in water.

Lappaconitin - [1a, 14a, 16b-trimethoxy-4- (N-acetyl-anthranoyloxy) -8, 9-Nethyl-18-noraconan].

The drug has antiarrhythmic effect. Refers to antiarrhythmic drugs of group I. Slows down the excitation of the atria, the bundle of His and the fibers of Purkinje. Does not have a noticeable negative inotropic effect, does not cause arterial hypotension.

Applied with supraventricular and ventricular ekstrasistolii, paroxysms, fibrillation and atrial flutter, paroxysmal supraventricular and ventricular tachycardia, as well as with arrhythmia in the background of myocardial infarction.

Assign inside, intravenously and intramuscularly.

Inside take 30 minutes before meals, washed down with warm water. Tablets are recommended to grind beforehand. First appoint 0,025 g every 8 hours. In the absence of effect, the dose is increased, assigning 0,025 g every 6 hours. A further increase in the dose to 0.05 g per reception, every 6 to 8 hours is possible. Higher doses for oral administration: single 0.15 g, daily 0.3 g.

Intravenously, allapinin is administered in a dose of 0.3-0.4 mg / kg. Before use, the drug is diluted to 20 ml with isotonic sodium chloride solution. Allapinin injected slowly - within 5 minutes. If necessary, after 6 hours, the drug administration at a dose of 0.3 mg / kg is repeated.

With intravenous administration, the action of the drug develops relatively slowly - after 15-20 minutes and reaches a maximum at 2 o'clock; The effect persists for a long time - up to 6 - 8 hours. (It is possible that this is due to the formation and accumulation of metabolites.)

Intramuscularly, allapinin is administered at a dose of 0.4 mg / kg every 6 hours.

When using alapinin, dizziness, headache, a feeling of heaviness in the head, face hyperemia, diplopia are possible. With the severity of these phenomena, the dose of the drug should be reduced. Sometimes there are allergic reactions.

The drug causes a slowdown in intraventricular conduction. May have arrhythmogenic effects. There may be a sinus tachycardia.

Contraindications: atrioventricular blockade of II degree and higher degrees, severe hepatic and renal insufficiency.
