Description of the medicine: Droperidolum (Droperidolum)

DROPERIDOL (DROPERIDOLUM). 1- {1 - [3-p-Fluorobenzoyl) propyl] -1,2,3,4-tetrahydro-4-pyridyl} 2-benzimidazolinone.

Synonyms: Dehydrobenzperidol, Dridol, Droleptan, Droperidol, Inapsin, Sintodril and others.

Light creamy crystalline powder. When stored in the air and in the light turns yellow.

Almost nerastvorim in water, little soluble in alcohol.

Neuroleptic agent from the group of butyrophenones. Has a fast, strong, but short action. The effect when injected into a vein manifests itself in 2 - 5 minutes, reaches a maximum after 20 - 30 minutes. Depending on the dose, the effect lasts up to 2 to 3 hours.

The drug potentiates the action of analgesic and hypnotics. Has anti-shock and antiemetic properties. Has a-adrenolytic effect; Blocks central dopamine receptors; Does not possess cholinolytic activity. Reduces blood pressure, has antiarrhythmic effect. Has a strong catalepogenic activity.

In psychiatric practice, droperidol is used in psychomotor agitation, hallucinations. It is mainly used in anesthetic practice for neuroleptanalgesia, usually in combination with fentanyl (see) or other analgesics.

The combined use of droperidol and fentanyl causes a rapid neuroleptic and analgesic effect, drowsiness, muscle relaxation, prevents shock, and has an antiemetic effect. Patients easily exit from the state of neuroleptic analgesia.

Apply droperidol for premedication, during surgery and in the postoperative period. Used for endotracheal anesthesia, as well as for operations with local anesthesia.

Introduce droperidol under the skin, intramuscularly and intravenously.

With premedication and neuroleptanalgesia, droperidol is usually administered intramuscularly for 30 to 45 minutes before the operation at a dose of 2, 5 to 5 mg (1-2 ml of 0.25% solution), together with 0.05 to 0.1 mg (1 to 2 ml 0.005 % Solution) fentanyl or 20 mg (1 ml 2% solution) of promedol. At the same time, 0.5 mg (0.5 ml 0.1% solution) of atropine or methacin is administered.

When endotracheal anesthesia is administered first droperidol at a rate of 2, 5 mg per 5 kg of body weight, together with fentanyl at the rate of 0.05 mg per 5 kg of body weight (corresponding to 1 ml of combined preparation of talamonal for 5 kg of body weight). Then for the introduction of anesthesia, use non-alcoholic anesthetics, sodium oxybutyrate or tranquilizers (diazepam, sibazon). Anesthesia is maintained by nitrous oxide with oxygen in a ratio of 1: 1 or 3: 1. During the operation, every 15 to 30 minutes or when signs of weakness appear, additionally fentanyl is administered at 0.05 to 0.1 mg (1 to 2 ml). In the case of the appearance of cardiac arrhythmias, additionally droperidol (2, 5 - 5 mg) is administered.

Muscle relaxants are used to relax the muscles.

During the injection of droperidol and fentanyl, the patient breathes a mixture of oxygen with nitrous oxide (1: 1).

With local anesthesia, droperidol is administered intravenously (slowly) or intramuscularly at a rate of 0.1 mg / kg.
