PROSTENON (Prostenonum).

The domestic preparation of prostaglandin E 2 - prostenon - is used in medical practice in the form of alcohol (0.1% and 0.5%) solution.

Prostenon, like dinoprost and dinoprostone, causes stimulation of the uterus musculature and is used for labor and stimulation of labor.

In addition, like other preparations of prostaglandin E 2 (see Prostaglandins), the prostenon has an antihypertensive effect. The hypotensive effect is associated with peripheral vasodilator activity. The drug expands arterioles and venules, increases renal circulation, promotes an increase in diuresis and sodium naresis, has antiaggregational effect; Also has bronchodilating activity.

In connection with the hypotensive effect, the prostenone has also been used to treat arterial hypertension.

As a means of stimulating labor activity, the prostenon is used in a hospital with the following conditions: late toxicosis, a pregnant pregnancy, a Rh-conflict situation, and in other cases, requiring early delivery of labor, including those accompanied by hypertension, cardiovascular insufficiency, Diabetes mellitus, bronchial asthma, as well as premature discharge of amniotic fluid, weakness of labor activity, if there are no contraindications due to the condition of the fetus and the mother (see Contraindications).

To use as a stimulant generic activity, dilute 1 ml of 0.1% (or 0.5%) solution of prostenone in isotonic sodium chloride solution. To do this, the contents of 1 ampoule are mixed with 500 ml of isotonic sodium chloride solution, and shaken. The resulting solution is suitable for use for 24 hours.

The prepared solution of the drug is introduced at a rate of 6 to 10 drops per minute. In the absence of pronounced effect after 20 - 30 minutes, the infusion rate is gradually increased to 20 drops per minute (add every 4 to 5 drops per minute) until the establishment of regular labor. The maximum rate of drug administration should not exceed 35 drops per minute. During the introduction of the drug, a dynamic observation of the nature of the labor and the state of the fetus is necessary. The latent period varies from 20 to 40 minutes. When hypertension of the uterus or deterioration of the fetus occurs, the infusion of the solution is stopped until the disappearance of these phenomena. The infusion of the prostenone solution is resumed; Halving the dose; In the future, the rate of administration can be gradually increased. Prostenone dosage should be individual, depending on the activity of the uterus and tolerability. The amount of the drug used varies from 0.75 to 5 mg.

As an antihypertensive agent prostenon used in cases that are resistant to the action of "normal" antihypertensive agents.
