Aprotinin.

Synonyms: Gordoks, Ingiprol, Ingitril, Contrikal, Pantripin, Trasilol, Trascolan, Contrical, Gordox, Ingiprolum, Ingitrilum, Pantrypinum, Trascolan, Trasylol.

An antifenzional drug obtained from the lungs or pancreas of cattle.

Inhibits proteolytic enzymes (trypsin, chymotrypsin, kallikrein and plasmin) in blood plasma.

Inhibiting plasmin, reduces the fibrinolytic activity of the blood, has a hemostatic effect with coagulopathy.

According to modern data, acute pancreatitis changes the activation of pancreatic enzymes (trypsin, chymotrypsin, lipases, etc.). Under physiological conditions, these enzymes are in the gland in a zymogenic (inactive) form and are activated only when entering the intestine. Violation of metabolic processes and the appearance of cytokinase (as a result of infections, injuries, stone formation, etc.) can lead to the release of activated enzymes in the pancreatic ducts, which causes self-digestion of the gland tissues, interstitial hemorrhages, swelling and other changes.

Assign for acute and chronic pancreatitis, pancreatic necrosis, for the prevention of autolysis of the pancreas, with operations on it and a number of located organs, if there is a danger of injury.

They are also used for hyperfibrinolytic bleeding due to hyperplasia, in open-heart surgery with the use of artificial circulation.

Possible side effects: arterial hypotension, tachycardia, nausea and vomiting (with rapid administration), bronchospasm, hallucinations, psychoses, allergic reactions, thrombophlebitis at the injection site.
Contraindications: disseminated intravascular coagulation syndrome (except for the phase of hypercoagulability), I and III trimesters of pregnancy, breast-feeding.

Incompatible with glucocorticosteroids and infusion solutions containing amino acids and fat emulsions; Reduces the activity of streptokinase, urokinase and alteplase.

Pantrypine (Pantrypinum). The drug is aprotinin, obtained from the pancreas of cattle.

Liofilizirovashshy powder yellowish color. Soluble in water.

The activity of pantripine is determined by the biological method for the ability to reduce the activity of trypsin. In 1 g of the drug contains not less than 650 units.

Applied for the treatment of acute pancreatitis and recurrences of chronic pancreatitis, the prevention of pancreatitis in stomach and bile duct operations if there is a risk of injury to the pancreas.

It should be noted that in severe cases, the use of the drug does not replace surgical intervention.

Enter intravenously. In severe forms of pancreatitis, 100-125 units are administered simultaneously in 10-20 ml of a 5% solution of glucose or an isotonic solution of sodium chloride.
