Phimosis, paraphimosis

Phimosis, paraphimosis. Phimosis is an innate elongation and constriction of the prepuce. There are two forms. 1. Hypertrophic phimosis - congenital "trunk-like" elongation and narrowing of the foreskin, under which recurrent inflammation easily arises, violation of urination leads to the development of a vicious circle and an increase in the size of the prepuce sac often with a simultaneous shortening of the frenum. 2. Atrophic phimosis - narrowing of the foreskin with age-related atrophy, scleroderma, or chronic balanitis without previous hypertrophy.

Differential diagnosis: physiological gluing of the inner leaf of the prepuce with the glans penis in young children. Do not divide by force!

Complications. Chronic balanoposthitis, pyelonephritis, cancer of the glans penis, sexual intercourse disorder, paraphimosis.

Treatment of phimosis - excision of the foreskin. The prognosis is favorable.

Paraphimosis is an impairment of the head of the penis retracted by the foreskin.

Complications: necrosis of the head of the penis.

Treatment: with a swollen glans penis after a five-minute cooling try to reponate the head. In case of failure - dissection of the restraining ring on the back surface and after disappearance of the edema - excision of the prepuce.
