Causes and mechanism of occurrence, course, diagnosis

Myocardial infarction is one of the manifestations of ischemic heart disease. This is a serious disease, characterized by necrosis (necrosis) of the site of the heart muscle, which is caused by a violation of its blood supply. This occurs as a result of a mismatch between the need of the heart in oxygen and the possibility of its delivery.

In 98% of cases, atherosclerosis of coronary arteries feeding the heart is important in the occurrence of myocardial infarction. The mechanism of occurrence of plaques and thrombi is described in the section "IHD". After the formation of the plaque, it gradually ulcerates, becomes covered with cracks, where the platelets rush. The plaque increases in size, a thrombus can form at this point in the artery lumen.

A plaque or thrombus can close the lumen of the vessel, a clot can also come off and the lumen of another large vessel (thrombosis) is closed. Coronary vessels can be covered with plaques on a large extent. In a number of cases, all three coronary arteries are affected, but there may be the presence of isolated plaques.

One of the following causes of the acute violation of the coronary circulation is vasospasm. There are cases of myocardial infarction without coronary artery atherosclerosis.

In the mechanism of infarction also play the role of changes in the properties of the blood - the enhancement of the coagulation function, the release of the stress hormone (adrenaline) into the blood.

Myocardial infarction more often affects men, but by the age of 50 the risk of this disease in women and men is compared. In recent decades, the infarction has significantly younger and is often observed in young people. Infarction is one of the causes of disability in cardiovascular disease.

Periods of myocardial infarction

There are 5 periods in the development of this disease: pre-infarction, acute, acute, subacute, postinfarction.

  • Pre-infarction period. It can last from a few minutes to 1.5 months. Usually at this time, angina attacks increase, their intensity increases. If timely treatment and prevention begin, a heart attack can be avoided.
  • The sharpest period. Appears suddenly. There are several variants of the clinical picture:
    • Painful (anginal). This is the most common variant, it accounts for about 90% of cases. In this case, the patient has severe pain behind the sternum, giving to the left shoulder, scapula, collarbone and lower jaw. The attack of pain can last up to 2 - 3 hours, but it happens that it lasts several days. The patient at the same time feels a sense of anxiety, depression, fear of death. To provide first aid in case of suspected myocardial infarction, it is necessary to take nitroglycerin (2 tablets under the tongue) with an interval of 5 minutes and any pain medication. At the same time, it is urgent to call an emergency team and deliver the patient to a specialized clinic;
    • asthmatic. The clinical picture resembles an attack of bronchial asthma - the patient experiences difficulty breathing, shortness of breath, chest discomfort. This variant is more common in elderly people and with repeated infarction;
    • abdominal. It begins with pain in the upper half of the abdomen, accompanied by nausea, vomiting, not bringing relief, rapid pulse and lower blood pressure. Sometimes this form is mistaken for an acute disease of the abdominal cavity (acute abdomen);
    • Arrhythmic. This variant is characterized by a sharp change in the heart rate - its rapidity (tachycardia), unevenness or sharp loss (atrioventricular blockade) and loss of consciousness;
    • cerebral. The leading symptom in this is a severe headache, which can be accompanied by a disorder of vision, a violation of consciousness. Often, this creates paralysis;
    • atypical. It is expressed in bouts of pain of diverse localization;
    • Asymptomatic. With this version of the flow, a person does not feel pain. An infarct is detected accidentally by an electrocardiogram and with special blood tests.
  • A sharp period. Its duration is 10 days from the time of the infarction. It is at this time that the necrosis zone is finally formed at the site of death of myocardial cells and scar formation begins. As in the acute period, the patient is shown a strict bed rest. Usually this period is accompanied by signs of an inflammatory process - an increase in body temperature, nausea, changes in blood tests that correspond to inflammation. During this period, serious complications such as shock, pulmonary edema, arrhythmias may occur.
  • Subacute period. It lasts about 8 weeks. At this time the scar is compacted, replaced by a connective tissue. The patient is allowed to switch to a semi-postal regime and perform elementary physical activities in the form of therapeutic gymnastics in the ward under the supervision of the instructor of therapeutic physical training.
  • Post-infarction period. It lasts six months from the start of the disease. The patient is transferred to a polyclinic regime. In the clinic, the patient undergoes rehabilitation stages under the supervision of specialists. During this period, the occurrence of a repeated infarction can not be ruled out. This can happen both at rest and during physical exertion. It can manifest as a repeated heart attack, heart failure or rest angina.

The diagnosis of this disease is established by the combination of several symptoms, the most important of which are typical pain sensations, changes in the electrocardiogram and in special blood tests indicating damage to the heart cells.

A classic pattern of pain in myocardial infarction is prolonged, intense, compressive pain behind the sternum, independent of the position of the body. It is accompanied by sweating, fear of death, more often observed in the morning hours. The pain does not go away after taking nitroglycerin.

Folk remedies for treatment of myocardial infarction

Phytotherapy

  • Recipe: 50 grams of leaves of wild strawberry, cinnamon.
    Ingredients mixed, pour 0.5 liters of boiled water, put on a heated water bath for 15 minutes, then remove and strain after complete cooling. Press the mass and bring it to the original volume with boiled water.
    Take 1/2 - 1/4 cup 2 times a day before meals.
  • Recipe: 20 grams of ground valerian roots, herbaceous leaves of the motherwort, cumin fruit, 1 cup of boiling water.
    Ingredients to mix. 1 tbsp. L. Collect pour boiling water, put in a water bath for 15 minutes. Infuse for 30 minutes. Strain, raw squeeze.
    Take 1 glass before bedtime.
  • Recipe: 20 g of hawthorn flowers, chopped valerian root, 15 grams of spring herrings, 1 glass of boiling water.
    Stir the ingredients carefully, 1 tbsp. L. Collect pour boiling water, insist 40 minutes in a warm place, drain, raw materials squeeze out.
    Take 1/2 cup 2 times a day in the morning and before going to bed.
  • Recipe: 2 parts of the flowers of the immortelle, sandy, tansy flowers, 1 part of elecampane high (roots), dandelion roots, 5 parts of St. John's wort, 3 parts of sage medicinal herb, 0.5 liters of steep boiling water.
    All components are mixed, 3 tbsp. L. Collect pour boiling water and insist in the thermos for 10 hours. Infuse the extract.
    Drink the infusion in 3 divided doses during the day, before meals.
  • Recipe: 5 grams of spring herrings, grassy jaundice greyish, flowers of sainfoin, 1 cup of boiling water.
    Collect the pour boiling water, simmer for 10 minutes, pour into a glass, insist until it cools.
    Take 1/2 cup 2 times a day for 30 minutes before meals. The course of treatment is 6 months.
  • Recipe: 25 grams of blackberry leaves with a bluish, grass leaves of the motherwort ordinary, 15 grams of mistletoe white leaves, herbage marshwort, 20 g of leaves of the ash-tree fragrant.
    Mix all ingredients, pour 0.5 liters of boiled water, put on a water bath for 15 minutes, then remove and strain after complete cooling. Bring boiled water to the original volume.

  • Recipe: 20 g cones of hops, yarrow herbs, valerian herbs, melissa leaves, corn stigmas, 1 cup of boiling water.
    2 tbsp. L. Collect pour boiling water, close the lid, insist on a water bath. Cool, drain, raw materials to squeeze.
    Take 1/2 cup 1 time a day in the morning 30 minutes before eating.
  • Recipe: 20 g of broad-leaved broad-leaved flowers, 10 g of sainfoin grass, 1 l of boiled water.

Cereals treatment

In germinated grains contains much more nutrients and trace elements than in ordinary grains. During the germination of grain, the amount of nutrients and biologically active substances in it increases several times. In addition, the sprouts contain trace elements. When using sprouts in human food, the human body spends far less effort in digesting and digesting them compared to products obtained from dry grain.

There are several ways to germinate the grain at home.

The simplest and most reliable method is the can. To do this, take a mayonnaise pot, grain (2/3 cans), gauze and a glass pan. The grain is pretreated and disinfected (0.25% solution of potassium permanganate, and then boiled water), 2/3 of can is poured and poured with water. Water should also be pre-cleaned and enriched with microelements, since the quality of the water directly affects the quality of the germinated grain.

After 10 to 12 hours, the remaining water is drained, wheat sprouts are laid out on a glass pan on which a four-layer gauze moistened with enriched water is lined.

Above, the grain is covered with 4 more layers of gauze and left for 2 days. Germs should reach a length of 1 cm.

To do this, you need a tray or tray (stainless steel, enameled or made of food plastic) with a wall height of 5 - 6 cm. At the bottom put a burlap.

The grain grows more actively in the light. For this purpose it is possible to arrange illumination by lamps of type "Flora". Grain (barley, oats, wheat, rye, etc.) is poured in a layer of 3 - 4 cm and kept at a temperature of 15 - 20 ° C for 3 to 4 days before the appearance of sprouts no longer than 5 mm. After laying the grain for germination, it should be covered with gauze and poured with water until the gauze gets wet. After the grain has swelled, the water should be refilled again. It is necessary to monitor the humidity of the grain daily. It should not dry out. Further sprouted grain is washed and eaten.

Traditional means of treatment of myocardial infarction

In the initial stages of myocardial infarction, the main treatment measures are aimed at reducing the pain syndrome and eliminating the complications that arise. Various pain medications are used, including narcotic substances. Also used are drugs that normalize blood coagulation, eliminating arrhythmia. By the end of the inpatient stage, the patient must fully service himself, climb himself on 1 - 2 flights of stairs and make short walks in several receptions during the day. In addition, doctors solve the problems of the psychological and pedagogical plan. Restoring health and working capacity is the main goal. It must be remembered that the scar after the infarction transferred can not disappear, and the heart has to learn how to perform its functions in the new conditions.

Success largely depends on the will, perseverance, patient activity and compliance with the recommendations of specialists. In addition to receiving medical treatment in the hospital, the patient must systematically engage in therapeutic gymnastics, reasonably tempered, and rationally fed. The main task is to accustom the heart to physical stress gradually. Means to achieve this goal are morning exercises, therapeutic exercises, dosed walking. Dosed walking is the most important part of exercise therapy. Walking is a natural way of moving a person. It has a training effect on the body and helps restore the functions of the heart muscle. Walking improves appetite, strengthens the respiratory muscles and increases the vital capacity of the lungs. In addition, walking calms the nervous system and causes positive emotions: cheerfulness, good mood. However, walking, like any muscular activity, is regulated by time, distance and tempo. The initial route for dosed walking is 500 - 1000 m at a rate of 70 - 80 steps per minute. Then, from the third week, the duration of walking can be increased every day by 200-300 m. After a month of regular exercise dosed walking along with increasing the distance, you can increase the tempo to 90-100 steps per minute. In the future, it is recommended to go from 4 to 8 km daily. During walking, you need to rest while sitting or standing (1 - 2 minutes). All exercises should be held under the control of the pulse and well-being.

Walking should be done in any weather, but in very cold or hot weather its duration can be reduced or replaced by exercises indoors. You must beware of cooling hands and feet.

Dosed walking should be started no earlier than 1 hour after a meal or at least 1 hour before breakfast. Before the start of walking, you should rest for 7 to 10 minutes, then count the pulse for 1 minute and start moving.

The cardiological diet includes many products used in atherosclerosis. It is the use of vegetables, fruits, rich in vitamins, bread with bran.

It is not recommended to use products that cause bloating and high diaphragm position.

Food is taken 5 to 6 times a day, dinner should be easy, no later than 3 hours before bedtime.

In addition, it is recommended to limit the intake of table salt to 5 g per day.

Cooking salt has the property of retaining water; In addition, its components in excess quantities may have an undesirable effect on the functioning of the damaged myocardium.

Patients are also advised to limit the use of liquid to 1000 - 1200 ml per day, including the first dish.

From the methods of physiotherapy in case of myocardial infarction, electrosleep with the purpose of general sedative effect on the body, hydrotherapy in the form of various therapeutic showers, 2- and 4-chamber carbonic, hydrogen sulfide, oxygen baths (for the limbs) are used.

Also, "dry" carbon dioxide baths are used.

In uncomplicated cases, the use of laser therapy for the heart area is effective.
