The invention relates to medicine in particular to obstetrics. Based on the data of ultrasonic fetometry, three parameters are determined: BDP (biparietal size of the fetal head in mm), DB (fetal thigh length in mm), and OJ (fetal abdominal circumference in mm). And according to the sum of products of these parameters, the coefficients of 16.980, 22.000, 0.007, respectively, determine the body weight of the fetus (M). M = 16,980 H BPR + 22,000 H DB + 0.007 H 2 . The method significantly improves the accuracy of determining the mass of large fetuses, which allows you to choose the right tactics for labor and reduce the risk of perinatal pathology. The method is simple, accessible and can be recommended for wide application in women's clinics and maternity hospitals.

DESCRIPTION OF THE INVENTION

The invention relates to medicine, namely obstetrics.

The analogs of this invention are:

1. Proposed by I. F. Jordania in 1950 and based on measuring at the end of pregnancy the circumference of the abdomen and the height of the uterine fundus above the bosom [1,21.

2. Method 3. S. Stroikova (1954), who modified the method of IF Jordania, introducing a constant in her, depending on the body weight of the pregnant woman [1, 2].

3. The method of AA Dobrovolsky (1970), which made it possible to determine the mass of the intrauterine fetus at any gestation period, taking into account the growth of the pregnant [1, 2].

4. AI Kruchem (1982) developed a method for predicting fetal body weight, based on the relationship between placental localization in the uterus, the initial mass of the body and the growth of the pregnant woman, identified by the author [1, 2].

The foregoing methods, despite the ease of use, do not allow a reliable prediction of the mass of a large fetus, which reduces their diagnostic value. The introduction of ultrasound methods of examination of pregnant women made it possible to more reliably predict the body weight of the intrauterine fetus.

The prototype of the invention is the formula of VN Demidov and co-authors [3], which includes several biometric indicators of the fetus:

M = 186.6 H G - 3490.3 H D 2 + 43.9 H A - 717.8 H A 2 + 615 H C + 243.8 H D + 17849.0; (1)

Where M is the mass of the fetal body in cm;

D - the size of the fetal head, calculated according to formula (2) in cm;

A is the diameter of the fetal tummy in cm;

C is the transverse dimension of the heart in cm;

D - length of femur in cm.

Where BDP is the biparietal size of the fetal head in cm;

LZR - frontal-occipital size in cm.

Demidov's formula allows to obtain satisfactory results in predicting the weight of fruits with average height and weight indices and / or a syndrome of intrauterine growth retardation, but with macrosomia the magnitude of the error is 10-12% [3, 4, 5].

To optimize the timing and method of delivery of pregnant women with a large fetus, a more accurate determination of the fetal body mass is necessary.

The essence of the invention.

The purpose of the invention is to improve the accuracy of determining the mass in macrosomia of the fetus. The method is based on ultrasonic fetometry in the term of full term pregnancy.

Define the following parameters:

BDP - biparietal size of the fetal head in mm;

DB - length of femur thigh in mm;

OJ - the circumference of the fetal tummy in mm.

In the mathematical processing of data, a decisive rule was derived for determining the mass of a large fetus:

M = 16,980 H BPR + 22,000 H DB + 0.007 H CL 2 (3)

The method allows you to accurately predict the mass of a large fetus.

Example 1. The history of genera N 668 for 1996. With fetal biometry, the following data were obtained:

BPR - 90 mm, LZR -104 mm, DB-78 mm, OZh-363 mm, C-47 mm.

Translating the obtained indices and units of measurement into the parameters required when working with the Demidov formulas (1 and 2), we obtain the following result:

T = (9.0 + 10.4) / 2 = 9.7 cm.



Having translated the obtained indices and units of measurement into the parameters required when working with formulas (3), we obtain the following result:

M = 16,980 h 90 + 22,000 h 78 + 0,007 h 363 2 = 4167 g.

At the birth of a child, the body weight is 4100 g.

Example 2. The history of the birth of N 1374 for 1996. With fetal biometry, the following data were obtained:

BPR - 94 mm, LZR - 107 mm, DB - 73 mm, OZh - 369 mm, C - 44 mm.

Translating the obtained indices and units of measurement into the parameters required when working with the Demidov formulas (1 and 2), we obtain the following result:

T = (9.4 + 10.7) / 2 = 10.05 cm.



Having translated the obtained indices and units of measurement into the parameters required when working with formulas (3), we obtain the following result:

M = 16,980 × 94 + 22,000 × 73 + 0.007 × 369 2 = 4155 g.

At the birth of a child, the body weight is 4200 g.

Thus, the proposed method significantly improves the accuracy of determining the body weight in large fruits, which allows you to choose the right tactics for labor and reduce the risk of perinatal pathology in the mother and fetus.

The method is available for use in term of full term pregnancy, it is simple and does not require additional measurements in comparison with traditional ones.
