The invention relates to medicine, in particular to obstetrics and gynecology. The method provides an increase in the sensitivity and specificity of diagnosis of purulent placenta. A blood test is performed on a pregnant woman, while the index of phagocyte activity is determined in the blood, and at a value from 0.44 to 0.49, purulent placenta is determined.
DESCRIPTION OF THE INVENTION
The invention relates to the field of medicine, namely laboratory diagnostics in obstetrics and gynecology.
There is a known method for diagnosing fetoplacental insufficiency, including purulent placenta, to determine the level of thermostable alkaline phosphatase in the blood serum of pregnant women according to the method of Bodansky (Kolb VG, Kamyshnikov BC Clinical Biochemistry .- Minsk, 1976. - P. 89-95).
Disadvantages of the prototype: complex, time-consuming, it takes a long time to complete.
The invention is directed to the solution of the problem: simplification with high accuracy, acceleration of the method.
These tasks are achieved by determining in the stabilized venous blood of pregnant women the index of activity of phagocytes (IAF) and at a value from 0.44 to 0.49 the purulent placenta is determined.
The method is carried out as follows. In the pregnant woman, the phagocytic activity of leukocytes is determined by the method of determining the deficiency of antimicrobial protection. As an index of phagocytosis determine the index of activity of phagocytes. IAF is calculated as follows. The ratio of the sum of objects absorbed by active phagocytes to the total number of inactive and inactive phagocytes is calculated, the obtained data are correlated with the mean level of IAF in the control group (group of women with physiological pregnancy) and compared with the normative value of the indicator. As the object of phagocytosis, erythrocytes of sheep (concentration 50-60 million per 1 ml) are used. Stabilize 400 μl of venous blood in a pregnant woman in 100 μl of a gluicir. Then 5 μl of stabilized venous blood is mixed with 5 μl of sheep erythrocyte suspension and the mixture is kept for 15 minutes in a thermostat at a temperature of 37 ° C., periodically shaking. Then, 10-12 parallel strokes with an area of 18 x 3-4 mm are made on a single slide. Smears are stained using the Romanovsky-Giemsa method. When microscopically smearing its area is conditionally divided into three parts and in each of them account for one third of the total number of detected neutrophils. The absorbent capacity of 100 isolated neutrophils is estimated. The percentage of phagocytic cells and the number of objects absorbed by each phagocyte are counted. Calculate the total amount of phagocytic objects and their average number by one neutrophil (phagocytic number). Phagocytes are divided by the degree of activity into 3 groups: inactive (zero), low-active (usually with the absorption of one object) and active. The index of activity of phagocytes is calculated and at its value in the range 0.44-0.49 the purulent placenta is determined.
Examples of specific implementation.
Example 1 . Pregnant K., 26 years old. Diagnosis: Pregnancy is 35-36 weeks. OAA. Gestosis of mild severity against a background of chronic pyelonephritis. Chronic intrauterine fetal hypoxia. The index of activity of phagocytes was 0.49 and the purulent placenta was diagnosed. A course of therapy with broad-spectrum antibiotics was conducted. The birth occurred through natural birthmarks in the 39-40 weeks period, the Apgar score on the Apgar scale was 8/9, the baby was born without signs of intrauterine infection. Histological examination of the placenta - conclusion: purulent placenta.
Example 2 . Pregnant R., 20 years old. Diagnosis: Pregnancy is 35-36 weeks. FPN. Chronic intrauterine fetal hypoxia. The index of activity of phagocytes was 0.44 and the purulent placenta was diagnosed. A course of therapy with broad-spectrum antibiotics was conducted. The birth occurred through natural birthmarks at 38 weeks, the Apgar score was 8/9, the baby was born without signs of intrauterine infection. Histological examination of the placenta - conclusion: purulent placenta.
Example 3 . Pregnant L., 28 years old. Diagnosis: Pregnancy 34-35 weeks. OASA. Pelvic presentation of the fetus. Chronic pyelonephritis. The index of activity of phagocytes was 1.1 and the diagnosis of purulent placental was not exposed. Pregnant women were delivered by caesarean section at 39-40 weeks (indication for surgery: pelvic frontal presentation), the baby's score on the Apgar scale was 8/9, the baby was born without signs of intrauterine infection. Histological examination of the placenta - conclusion: purulent placental not revealed.
The positive effect of using the proposed method is as follows: the method is simple, does not require expensive reagents and specially trained personnel, the study time is 15-20 minutes. The method is highly specific and informative.
