The invention relates to dentistry, in particular to the diagnosis of dental diseases in children. This method provides for X-ray examination of the examined tooth and diagnosis based on the presence of pathological changes in the periodontal tissues revealed on the X-ray of the tooth. In the presence of pathological changes in the periodontal tissues, the values ​​of the optical density of the bone tissue of the tooth in the area of ​​bifurcation of the roots along the longitudinal axis, coinciding with the bifurcation axis and passing to the cortical plate of the rudiment of the permanent tooth, along the transverse axis passing through the centers of the roots of the tooth . The obtained values ​​are compared with the threshold value of the optical density, which is the value of the optical density equal to 0.125. In the case where there is an optical density value higher than the threshold value among the obtained values, chronic pulpitis of the tooth is diagnosed. Use of this method will increase the objectivity of diagnosis of chronic pulpitis of baby teeth.

DESCRIPTION OF THE INVENTION

The invention relates to dentistry, in particular to the diagnosis of dental diseases in children.

Objective and timely diagnosis of chronic pulpitis of infant teeth in children is of fundamental importance for the selection of an adequate method of treatment and the limit of surgical intervention.

Traditional is the diagnosis of this disease according to the medical examination and the subjective feelings of the child, which he, as a rule, can not accurately convey. The correctness of the diagnosis based on these data is largely dependent on the experience and professional training of the attending physician, which makes this method of diagnosis inadequate objective.

To increase the accuracy of diagnosis of chronic pulpitis of the baby teeth, along with the data of the clinical examination, it is desirable to involve data from objective apparatus research.

In connection with the development of modern X-ray equipment, which allows to reduce the dose radiation burden on the patient and at the same time to obtain multifaceted results of the survey, the application of the data of X-ray study in the diagnosis of dental diseases in children is very promising.

So, there is a known method for diagnosing chronic pulpitis of baby teeth in children [VM Elizarova et al., "Features of diagnosis and X-ray examination in chronic forms of pulpitis of baby teeth in children," "Institute of Stomatology", 2002, No.1, pp.14-16], which was selected by the authors as the closest analogue.

This diagnostic method includes radiographing of the examined tooth and subsequent analysis of the obtained radiograph. On the roentgenogram of the examined tooth, qualitative signs of pathological changes in periodontal tissues are revealed, in particular, weakening of the pattern of ossicles, thinning and osteoporosis of the compact plate of the alveoli, widening of the periodontal gap in the bifurcation area of ​​the roots. When these signs are detected, the child is diagnosed with chronic pulpitis of the milk tooth.

The use of X-ray examination in this method makes it more objective, however, since in this method the diagnosis is carried out only on qualitative criteria, it is still quite subjective.

The objective of the claimed invention is to increase the objectivity of the method for diagnosing chronic pulpitis of infant teeth.

The essence of the invention consists in the fact that in the method for diagnosing chronic pulpitis of infant teeth, which includes the x-ray of the examined tooth and the diagnosis, taking into account the presence of pathological changes in the periodontal tissues revealed on the x-ray of the tooth, according to the invention, in the presence of pathological changes in the periodontal tissues, Density of the bone tissue of the tooth in the root bifurcation region, the values ​​obtained are compared with a threshold value of the optical density, which is the value of the optical density of 0.125, and in the case where the optical density exceeds the threshold value, the chronic tooth pulpitis .

It is expedient to determine the optical density of the bone tissue of the tooth in the area of ​​the bifurcation of the roots along the longitudinal axis, coinciding with the bifurcation axis and passing to the cortical plate of the anterior tooth rudiment, and along the transverse axis passing through the middle of the root of the tooth.

The inventors of the claimed invention proposed in addition to using a quantitative parameter characterizing the state of the bone tissue of the tooth in the region of root bifurcation during the diagnosis of chronic pulpitis of infant teeth. As the indicated parameter, the authors chose the value of the optical density of the bone tissue of the tooth in the bifurcation area of ​​the roots, which is determined from x-ray data.

At the same time, based on the processing of a large number of radiographs of infant teeth, the authors found a threshold value of this quantitative parameter, which was 0.125. Studies have shown that if the X-ray of the tooth shows pathological changes in the periodontal tissues and among the bone density values ​​obtained from the x-ray data in the area of ​​the bifurcation of the tooth roots, there is a value exceeding the threshold value, this indicates a chronic pulpitis of the milk tooth.

It is most convenient to determine the value of the optical density of the bone tissue of the tooth in the root bifurcation region by estimating the change of this parameter along the longitudinal axis coinciding with the bifurcation axis and passing to the cortical plate of the rudimentary tooth anterior and along the transverse axis passing through the mid-roots of the tooth.

The proposed method is carried out as follows.

Conduct a medical examination of the patient, during which the signs indicating the possibility of chronic pulpitis in the patient.

X-ray examination of the tooth is performed.

The presence of pathological changes in periodontal tissues of the tooth to be examined is determined on the roentgenogram, in particular, such as weakening of the pattern of ossicles, thinning and osteoporosis of the compact plate of the alveolus, widening of the periodontal gap in the bifurcation area of ​​the roots, and the presence of carious cavities communicating with cavities filled with pulp.

When the pathological changes in the periodontal tissues are detected, the optical density of the bone tissue of the tooth in the area of ​​the root bifurcation is determined from x-ray data. The obtained values ​​of the optical density are compared with a threshold value equal to 0.125. In the event that at least one optical density value exceeding the threshold value is detected in the study area, chronic pulpitis of the milk tooth is diagnosed.

The possibility of implementing the method is shown in the example of a particular implementation.

Example.

Patient M., 6 years old, complained of getting food between 85 and 84 teeth.

Objectively: the mucosa is pale pink, on the medial surface of the 85 tooth and on the distal surface of the 84 teeth, deep carious cavities filled with a large amount of softened dentin are found. Reaction to temperature stimuli is positive, percussion is negative, palpation of the transitional fold is painless.

Radiologic examination of 85, 84 teeth was performed, for which the intraoral dental radiographs of the patient teeth were removed using the Trophy radiovisiograph with computer processing of the digital radiographic image.

X-rays revealed visible pathological changes in the periodontal tissues, suggesting the presence of chronic pulpitis of the examined teeth, namely: carious cavities communicate with the medial horn of the tooth pulp 85 and the distal horn of the tooth pulp 84.

To determine the diagnosis for each of the teeth, radiographic data determined the optical density of bone tissue in the bifurcation area of ​​the roots. The optical density was determined using densitograms obtained with the help of the radiophysiograph Trophy in the dynamic densitometry mode showing a change in the brightness of the x-ray image of the tooth along the longitudinal axis coinciding with the axis of bifurcation and passing to the cortical plate of the anterior tooth rudiment and along the transverse axis passing through the middle of the root of the tooth . At the same time, the brightness values ​​displayed on the densitograms were converted into units of optical density according to a known mathematical relationship connecting the indicated quantities. Among the obtained optical density values, a value of 0.286 was found which exceeds the threshold value of 0.125.

Was diagnosed: Chronic pulpitis 85, 84 teeth.
