DESCRIPTION OF THE INVENTION

The invention relates to medicine, namely to dermatology, and is intended for the treatment of papillomas of the skin.

Papilloma is a benign tumor of the epithelium of unclear etiology, characterized by papillary growth of the dermis, pronounced hyperkeratosis and acanthosis. Skin papillomatosis is the process of formation of multiple papillomas with localization on various parts of the body, more often on the face, neck, in the axillary and inguinal areas. Papillomatosis is accompanied by many concomitant diseases of various organs and systems, including viral etiology (ARVI, herpes, etc.). Relapses of the disease quite often occur (M.M. Vasiliev, 1999, V.Dubensky, R.Redko 2003; T. Haenaguch et al., 2002, etc.). To date, there are no radical measures to cure papillomatosis of the skin and prevent the recurrence of this disease (VN Zaets, 2001). With the prolonged existence of papillomas, malignant degeneration of benign tumors in 2.2% is possible (GG Nuriev et al., 1988).

Our immunological examination of patients with skin papillomatosis established the following signs of immune deficiency: a decrease in the level of interleukin 4 and the concentration of immunoglobulins A and E compared to the control group, an increase in gamma-interferon content, IgM and G, an increased level of circulating immune complexes (CIC) of low molecular weight 7%) and the reduced level of large-scale CEC (3.5%). This indicates that in patients with skin papillomatosis the function of the skin is reduced, cellular immunity is compromised, which is expressed in the sensitization of clones of T cells, which produce a significant amount of gamma-interferon. In addition, in the body of patients with skin papillomatosis, autoimmune reactions develop that contribute to chronic disease.

Until now, the treatment of papillomas of the skin has been carried out by a destructive method, involving the excision of elements, electrocoagulation, removal with a neodymium laser (Yu.K. Skripkin, V.N.Mordovtseva, 1999). As our observations showed, this method did not guarantee the cure for papillomatosis of the skin and relapses of the disease occurred at a frequency of 1 every 4 years - once every 2 months.

The aim of the invention is to prevent the recurrence of skin papillomatosis in persons with immune deficiency.

The goal is achieved by carrying out immunocorrection with the introduction of immunotropic drugs (polyoxidonium or immunofan according to indications) followed by immunostimulating herbs collection (buckthorn bark, dandelion root, parsley fruit, fennel fruits, peppermint leaves).

The method is carried out as follows

After a clinical examination of a patient with papillomatosis of the skin with a burdened course of the disease (high prevalence of the process, the presence of recurrences of papilloma formation, concomitant viral and bacterial infections with frequent exacerbation, allergic background), an immunological examination (immunogram) is performed. Further, the removal of papillomas by a destructive method (electrocoagulation followed by treatment of the site of destruction with a 5% solution of potassium permanganate). According to the results of the study of the immune status, one of the immunotropic preparations of polyoxidonium or immunophane is prescribed according to the indications. After the end of treatment, immunomodulators are prescribed immunostimulating herbs collection. Clinical follow-up of patients is carried out after 1, 6, 12 months. The absence of recurrence of the disease in these terms indicates the effectiveness of treatment.

For the observation, 39 people were taken. Of these, 18 with the typical course of skin papillomatosis were treated by electrocoagulation with papillomas and 21 patients with papillomatosis of the skin with complicated course (multiple and common papillomas and severe concomitant diseases), besides destructive therapy, immunomodulators were obtained. The age of the patients varied from 25 to 69 years, and the average age of patients in the allocated groups did not differ significantly. In most patients, the localization of papillomas was common and covered the face, neck, trunk, large folds of the skin. The number of papillomas was from 10 to 300. In the group of patients with unhealthy course, the average number of papillomas was significantly less than in the group of common papillomatosis. For patients with skin papillomatosis, the following accompanying diseases are characteristic: ARVI, herpes infection, chronic inflammatory processes with a frequency of exacerbations from 1 time in 2 years to 1 time in 2 months, allergic diseases. Patients treated with immunomodulators were divided into two groups: the first group, with the use of polyoxidonium (of 10 people); The second group - treated with immunophane (11 people). The table presents the results of the effectiveness of treatment of skin papillomatosis in patients with a typical and complicated course of the disease.

In the group of patients treated by electrocoagulation without the appointment of immunomodulators, recurrences of papillomas occurred in 13 of 18 (72.2 + _10.6%) under observation from 1 month to 1 year. Patients treated with polyoxidonium relapsed in 1 out of 10 people (10 + _9.5%) within 3 months after treatment, and there were no relapses during the rest of the period. In the group treated with immunophane, relapse occurred in 1 of 11 patients (9.1 + _8.6%) after 6 months.

Thus, the effectiveness of treatment with immunomodulators was higher in comparison with the application of the destructive method for polyoxidonium 7.2 times (t = 4.4, p <0.01) and for immunophane 7.9 times (t = 4.6; P <0.01), i.e. There is a significant difference in the effectiveness of treatment with and without immunocorrection.

The achievement of the effect of the invention on the prevention of skin papillomatosis is due to the following.

Immunotropic preparation polyoxidonium is a derivative of N-hydroxy poly-1,4-ethylene-piperazine, the main mechanism of immunomodulating action is the effect on phagocytic cells and natural killers, stimulation of antibody formation. The immunostimulatory harvest of herbs prescribed after the course of polyoxidonium acts synergistically and increases the activity of the main parts of the immunological surveillance systems. Immunofan correlates the immune and oxidation-antioxidant systems of the body in 3 phases. According to the chemical composition, this is a hexapeptide (arginyl-alpha-aspartyl-lysyl-valyl-tyrosine-arginine). During the middle phase, lasting 7-10 days, the reactions of phagocytosis increase; During the slow phase, the parameters of cellular and humoral immunity are restored. The preparation, in particular, stimulates the formation of IgA.

Thus, the effect of preventing recurrences of papillomas of the skin is achieved by a complex effect on the immune system of immunomodulating drugs in combination with plant immunostimulants prolonging their action.

For the first time the scheme of treatment of papillomatosis of the skin with weighed flow providing a pronounced anti-relapse effect is substantiated.

The method is carried out as follows

After removal of the papillom of the skin with the help of electrocoagulation and treatment of the site of destruction with a 5% solution of potassium permanganate, patients with immunocompromentation are prescribed polyoxidonium or immunophan according to the indications.
