The invention relates to medicine> namely, to dermatology, and relates to a method of treating psoriasis. The invention provides increased therapeutic efficacy and prevention of side effects. The essence of the method is that, in parallel with the photochemotherapy, vitamin D2 is administered 1000 IU 3 times a day and vitamin A is administered at 3300 IU 3 times a day for three weeks.
DESCRIPTION OF THE INVENTION
The invention relates to medicine, namely, to dermatology and relates to a method of treating psoriasis.
Known is a method for treating psoriasis with tigazone in combination with formiochemotherapy (Sov.Med. 1987, No. 8, pp. 77-80).
The essence of the method is the admission of tigazone at 75 mg per day for 14 days followed by photochemotherapy in a 4-day procedure with the reception of a photosensitizer puvalen and simultaneous reduction of the dose of tigazone to 50 mg per day to the state of clinical recovery.
However, it should be noted the resistance of the psoriatic process to the admission of tigazone and the use of photochemotherapy in approximately 6% of patients with the most severe forms of psoriasis, which significantly reduces the effectiveness of treatment.
The aim of the proposed method of treatment is to increase the therapeutic efficacy of the most severe and torpid forms of psoriasis and to eliminate side effects.
The goal is achieved by the fact that in parallel with the phytochemotherapy on a 4-day procedure using the Puvalen photosensitizer orally, vitamin D 2 1000 IU 3 times a day and vitamin A at 3,300 IU 3 times a day are administered peroral for three weeks.
Vitamin D 2 is involved in epidermal proliferation and differentiation. The proposed complex method of treatment potentiates the role of vitamin D 2 , i.e. A synergistic effect is achieved that provides a higher therapeutic level of treatment compared with the prototype, vitamin A provides the processes of keratogenesis in the epidermis.
The method is carried out as follows. Photochemotherapy is carried out 4 times a week according to the generally accepted method: after taking food, the patient is prescribed a puvalene photosensitizer at a rate of 0.6 mg per 1 kg of body weight and after 2 hours, ultraviolet irradiation with a wavelength of 320-360 nm is performed.
In parallel, 2 tablets (1 dragee - 500 IU) of vitamin D 2 ergocalciferol are prescribed 3 times a day and 1 dragee (1 dragee 3300 IU) of vitamin A (retinol acetate) 3 times a day after meals, for 3 weeks.
Outwardly use a combination of 2% salicylic ointment and cream Unna.
This method was used in the treatment of patients with psoriatic erythroderma, arthropathy, exudative, post-stage forms of the process.
The regression of the disease began with 2-3 days of therapy in the form of obvious palliation of the foci, a certain reduction in the dryness of the skin.
Clinical recovery was noted in 100% of patients.
The state of clinical recovery was characterized by the disappearance of psoriatic foci, the pronounced pigmentation of the skin and, to a greater extent, the former psoriatic foci. Side effects of complications after this course of treatment were not noted. Given the known influence of vitamin D 2 on the phosphoric calcium metabolism, the serum level was determined in the dynamics of treatment. The latter did not exceed the limits of the norm of 2.25 - 2.75 mm l / l.
Example 1 . Patient H., 62. He suffers from psoriasis with 32 years. There are annual, repeated, 2 - 3 months duration extremely torpid exacerbations of the psoriatic process. With regard to this long (4 months) exacerbation of the process in the form of psoriatic erythroderma, the treatment was performed in a hospital in the manner suggested.
The onset of regression was observed on the third day of therapy in the form of diminished brightness and flattening of the foci. The patient will receive 14 sessions of photochemotherapy, a total dose of vitamin D2 42000 IU, vitamin A - 140,000 IU, was discharged on the 29th day of hospitalization in a state of complete clinical remission. Side effects and complications after treatment were not noted. The serum calcium level, within the limits of the norm, is 2.61 mmol / l. After the treatment the remission lasts for the 5th month.
Example 2 . Patient R., 38 years old. Suffered from psoriasis for 13 years. Relapses are annual, in the form of psoriatic erythrodermia lasting 3-4 months. Traditional therapy is practically not effective, the course of the disease during the last exacerbation of the psoriatic process, and in the form of erythroderma. The patient was given a course of FTX according to a 4-day schedule with a photosensitizer puvalan, simultaneous intake of vitamin D 2 2 tablets 3 times a day, vitamin A 1 dragee 3 times a day. Regression was noted from the 4th day of therapy (3 FTX procedures and 4 days of vitamin D2, A) in the form of an obvious blanching of the foci of the foci, their flattening. The process completely regressed after 15 procedures of FHT and 3-week administration of vit. D 2 and A. Treatment was not accompanied by side effects, the occurrence of complications, the level of serum calcium in the normal range is 2.65 mmol / l.
Remission lasts for the 4th month.
At the beginning of the second week of therapy, marked pigmentation of the skin, marked by the end of treatment, was noted. The proposed method of therapy, we used to treat 18 patients with psoriatic erythroderma, arthropathy, exudative, pustular forms of psoriasis and extremely torpid forms of ordinary psoriasis. Clinical remission was achieved in all 18 patients.
Thus, the proposed method of treatment allows to achieve a clinical effect in 100% of cases of the most severe variants of the psoriatic process, to prevent side effects.
The method is effective clinically, is simple and accessible, it is feasible in outpatient settings.
