The method simplifies treatment, shortening and lengthening of relapse treatment of disease remission.

DESCRIPTION OF THE INVENTION

The invention relates to medicine, namely, dermatology, and can be used in the treatment of eczema and psoriasis.

Difficulties etix treatment of diseases associated with persistent chronic course, and tendency to relapse, due to their etiopathogenesis.

Recent studies oppedelili value and location neyroallergicheskih immunological, metabolic changes in the body of patients with eczema (Kubanova AA, et al Journal of Dermatology 1985 - 8. C. 16-19;.. Smolyagin et al Materials for the study of the pathogenesis and treatment of patients. .... eczema and neurodermatitis Regional scientific-practical conference dermatovenerologists Abstracts Orenburg, 1994. 10 pp .; Lobanovsky GN, Kravchenko LA dermatology Journal, Beauty and sexual pathology, 1998. -. 1 - p 107-110).

Attention is drawn to the value of neuropsychiatric disorders in the development of dermatoses, in particular the prevalence of inhibitory processes in the cerebral cortex, in connection with which, apparently, eczema occupies a central place in the structure of skin diseases in the elderly (Belousov TL Materials 4th All-Russian Congress of dermatologists Krasnodar 1976 - pp. 128-129, Kochergin NG c coavt Patologogeneticheskie mechanisms of atopic dermatitis Ref Honey Well -..... 1987 - 8 - c 6-10 .; ANTONIO DA Prohorenkovs VV Journal of dermatology 1995 -. 2. - P. 20-22).

The attention of researchers and attract the immune system in the pathogenesis of eczema. We examined patients showed immunodeficiency flowing hyperresponsiveness with a T-system and the B-cell population. It has been established a significant decrease in the number of subpopulations of helpers, while the production of suppressor remains at a normal level. This is particularly evident in elderly patients (Abidov MM et al 1982. Journal of Dermatology - 2. - P. 33-36;.. Daniel A. et al Abstracts - Dnepropetrovsk, 1991. - With... . 63; Novikov GM, GL Ivashkevich Abstracts, Ekaterinburg, 1994. pp 68-69)...

In recent years, actively developing methods for the treatment of chronic dermatoses with regard to their etiopathogenesis: antihistamines, anti-inflammatory drugs nesteroidnyh, vasoactive drugs, corticotropin in combination with topical therapy (Antonov AA, et al Journal of Dermatology - 1982 - 5 -.. pp 12-14; Shahtmeyster IJ, Shimanov NA Journal of dermatology - 1998 - 2 - pp 27-31;. Samsonov VA, et al Journal of dermatology - 1998 - 4... - S. 48-50).

A method of treating atopic dermatitis, including the study of the immune status of the patient, and with a decrease in the relative number of T-lymphocytes, increase in IgG and IgM, reduced IgA - the introduction of ascorbic acid and levamisole, alternating between taking the drug for 3 weeks (RF Patent 1638852, A 61 K 39/00, 27/09/95, BI 27).

The method allows to reduce the time of treatment of children with atopic dermatitis, prolongs remission of the disease, reduce the number of complications and relapses. The disadvantage of this method is its complexity and duration.

There is a method of treating psoriasis, which involves the study of the immune status of the patient and, if not the content of T-suppressors of T-helper cells, intramuscular injection, 2 times a day administered delargin within 5-10 days and the lack of normalization of the immune status - repetition rate (with A. . USSR 1821217, A 61 K 37/02, 06/15/93, BI 22).

However, the known methods are not sufficiently effective lecheniya, difficult to implement, durable, do not reduce the tendency to relapse, due to the lack of a comprehensive impact on the body of the drugs (central, immune, endocrine system). In addition, many side effects of drugs called.

The object of the invention is to find a therapeutic agent, simultaneously correcting endocrine, immune and other systems.

Technical result of the implementation of the invention manifests itself in the simplification of treatment and shortening its terms, lengthening interrecurrent period, which has an economic effect.

Said technical result is achieved in that the therapeutic agent is a synthetic analogue of the hormone of the pineal gland, synthesized from plant-derived amino acids at a dose of 0.5-1.5 mg, prior night's sleep, orally, for 7-20 days.

Known commercial drug is a synthetic analogue of the hormone of the pineal gland - melaxen, melatonin (manufacturer Melanin, USA) (RS - Dr. M., 1999, pp 600-601).

Pharmaceutical effect of the drug - adaptogenic, promotes the organization of the biological rhythm of the body and the normalization of nocturnal sleep. It stimulates cellular and humoral immunity, regulate neuroendocrine function.

Melatonin has an anti-stress effect, which is due to its effect on neurotransmitter systems, the impact on the timing of the circadian rhythm. Melatonin interferes with the activity of opioid mechanisms (Kumar MS, Chen S.Z., Sharp D. S. et al Diurnal Huctuation in melatonin enkephalin levels in the hypothalamus and preoptic areas of die male rat: Effects of pinealectomy // Neuroendocrinology - 1982.. . - Vol 35. -. P. - 28-32).

Acting simultaneously on neyroendokrinnuyu and the immune system, melatonin homeostasis optimizes and protects against stress (Maestroni J. Melatonin and the immune system // Melatonin and the Pincal Gland -. Paris, - 1992. - P. 59).

The figure shows the place of opioid peptides (white arrows) in vzoimootnosheniyah between the pineal gland (black arrows - the effect of melatonin), central nervous, endocrine and immune systems in the process of formation of hazardous reactions to stress (Clinical Medicine, 12, 1998, p. 49).

Indications for use are sleep disorders, adaptation of the body when changing external factors.

Research on the use of melatonin allowed to establish its beneficial effect on the normalization of the status of T cells, the thymus (Walter Pierpaolo William Reolson. The miracle of melatonin. M. Bean, 1997, S. 111).

In this case the recommended dosage of melatonin increases depending on age with 0.5-1 mg aged 40-44 to 3.5-5 mg 75 years and older.

The claimed combination of features provides the technical result, which is due to the above properties of melatonin.

Oral drug bedtime optimizes its operation by taking into account biological rhythms in the body, since it is known that melatonin is produced by the pineal gland during the night. The dose received by means of 0.5-1.5 mg for 7-20 days, depending not governed by age and severity of the disease, is necessary and sufficient to achieve a technical result, with the exception of side effects.

The possibility of practical implementation of the claimed method is illustrated by examples of its specific use on patients.

Example 1. Patient M., 70 years old, was admitted to the hospital with a diagnosis of microbial eczema right lower extremity complicated allergid, lymphadenitis.

Sick 3 years. The process is difficult, sharpening 3-5 times a year, normally for a period of remission 2-3 months. Exacerbations skin process are always prevalent, differing torpid to therapy. Comorbidities: chronic gastritis, colitis. Hypertensive heart disease 2 stages.

Regarding the previous exacerbation was treated in hospital and outpatient residence conventional means without effect, and therefore has been hospitalized in the regional clinical dermatovenerologic dispancep.

When a pathological process was prevalent from the trunk and limbs of the skin lesion. The main center was located on the right ctope and calf skin was greatly swollen, infiltrated and hyperemic, against this background, noted krupnoplastinchatoe scaling, exudative scales. The boundaries of a clear defeat, on the periphery of the expression screenings and erosive papular rash. Similar rashes absently located on the skin of the upper limbs and torso, longer at the back.

General biochemical blood parameters were no deviations from the norm.

The patient received treatment with melatonin 0.5 mg at night for 10 days. Externally on the lesions - cream Unna.

As a result, traveled on day 3 of treatment stopped itching, rash on the trunk and upper limbs disappeared. 5 To day the main focus on the left foot and shin dropped. Peeling foci ceased to 9 day of treatment, lesions completely regressed to the 11th day.

Clinical remission lasted 11 months and increased in comparison with the usual terms of 3 times.

Example 2. Patient K., 41 years old was admitted to hospital with a diagnosis of psoriasis krupnoopatovy common in stationary stage.

Sick 12 years. The process is difficult, sharpening up to 5 times a year, normally for a period of 2-3 months pemissy. Exacerbations skin process are always prevalent, differing torpid to therapy. Comorbidities: essential hypertension grade 2, cholecystitis.

Regarding the previous exacerbation was treated in hospital and outpatient traditional means, the effect is negligible, and therefore was re-admitted to the hospital.

When a pathological process was prevalent. On the scalp, trunk, limbs localized many large papular elements pozovogo bright colors. In the trunk there was a solid lesion, covering the skin of the back, waist, chest and abdomen. Across the surface of foci it was significantly expressed cepebristo-beloe peeling.

The patient received treatment with melatonin 0.5 mg at night for 10 days against the backdrop of external treatment cream Unna.

As a result of the treatment on day 3 foci pale to 5 day decreased infiltration, on the 7th day on the trunk in the solid portions of the lesion appeared healthy skin. Peeling foci stopped a 10 day treatment. Fully lesions regressed to the 13th day.

Clinical remission lasted 9 months and increased in comparison with the usual terms of 3 times.

Under the conditions of the Stavropol regional STI clinic claimed method used in the treatment of 45 patients with eczema and 15 psoriasis patients with different severity of the disease and in vozpacte from 36 to 82 years. Comparative data on the results of the treatment of the claimed and traditional methods are presented in Tables 1 and 2.

Comparative data traditional method of treatment and melatonin are presented in Table 1 for eczema, in Table 2 - for psoriasis.

Thus, the use of the claimed Cposob provides significant increases in efficiency of treatment of chronic dermatoses, reduction of material costs by lengthening the period of remission and reduction of time of stay of patients in hospital and can be recommended in clinical practice.
