Description of the medicine: Medilazid (Medilasidum)
Crystalline individual cardiac glycoside obtained semi-synthetically by digoxin methylation. It contains an additional methyl group in the sugar residue (D-digitoxosis).
Synonyms: Bemecor, b-Methyldigoxin, Bemecor, Bemekor, Digicor,
Crystalline powder, white or white with a slightly yellowish tint.
It is practically insoluble in water, difficultly in alcohol.
By biological (cardiotonic) activity, medilazide is close to digoxin; 1 g of the drug contains 2100 - 2700 GED or 2200 - 2900 KED.
According to the indications for use, it is close to digoxin.
Distinctive pharmacokinetic features of medilazide are rapid absorption when administered orally (after 5 - 25 minutes); cardiotonic action reaches a maximum in 50-60 minutes after administration. With intravenous administration, the effect begins to appear after 1 - 5 minutes. Compared with other digitalis glycosides, the drug has less cumulation; after cancellation, the duration of its exposure is 3 - 9 days, digoxin - 6 - 9, and digitoxin - 14 - 21 days. ”
Applied with various forms of chronic heart failure, with a constant form of atrial tachyarrhythmia, paroxysmal flicker and atrial flutter, supraventricular paroxysmal tachycardia.
Medilazide is prescribed orally for adults (regardless of food intake).
The dose is selected strictly individually. There are 3 ways of digitalization: fast, moderately fast and slow. To obtain a quick therapeutic effect, fast gradual saturation with medilazide is used for 24 36 hours (fast type of saturation). In this case, the initial dose is 0.8 - 1.0 mg, followed by the addition of 0.1 - 0.15 mg every 5 to 6 hours to obtain the optimal therapeutic effect. After that, they switch to maintenance doses (0.1 - 0.4 mg per day).
For moderately fast digitalization, the drug is prescribed in the first - second - third days at a dose of 0.6 - 0.8 mg (0.2 mg 3-4 times a day), in the fourth, fifth and sixth - 0.4 - 0 6 mg (0.2 mg 2-3 times a day).
Then they switch to maintenance doses - 0.1 to 0.4 mg (on average 0.2 mg per day).
For slow digitalization, medilazide is given for 10 days at a dose of 0.4 - 0.6 mg (0.2 mg 2 to 3 times a day), then they switch to maintenance doses (0.1 - 0.2 mg per day) )
The highest daily dose is 1, 25 mg.
By side effects and contraindications, it is close to other digitalis drugs there.
Release form: tablets of 0.0001 g (0.1 mg) in a package (test tubes) of 50 pieces.

The foreign drug Bemecor corresponding to medilazide is also available in the form of a solution in 2 ml ampoules (0.2 mg of β-methyldigoxin each), and in the form of drops for oral administration containing 0.6 mg of β-methyldigoxin in 1 ml (45 drops). Ampoules are used if necessary, quick saturation.
