Medication Description: Medilazid (Medilasidum)
Crystalline individual cardiac glycoside obtained semi synthetically - by digoxin methylation. Contains an additional methyl group in the sugar residue (D-digitoxose).
Synonyms: Bemecor, b-Methyldigoxin, Bemecor, Bemekor, Digicor,
White or white with a slightly yellowish sheen crystalline powder.
Practically insoluble in water, it is difficult - in alcohol.
According to the biological (cardiotonic) activity, medilazide is close to digoxin; 1 g of the preparation contains 2100 - 2700 HED or 2200 - 2900 KED.

Distinctive pharmacokinetic features of medilazide are rapid absorption by ingestion (after 5 - 25 minutes); cardiotonic action reaches a maximum after 50–60 min after administration. When administered intravenously, the effect begins to appear after 1 - 5 minutes. Compared with other digitalis glycosides, the drug has less cumulation; after the cancellation, the duration of its exposure is 3–9 days, digoxin - 6–9, and digitoxin - 14–21 days'.
Applied with various forms of chronic heart failure, with a constant form of atrial tachyarrhythmias, paroxysmal atrial flutter and atrial flutter, supraventricular paroxysmal tachycardia.
Medilazide is prescribed for adults (regardless of the meal).
The dose is selected strictly individually. There are 3 ways to digitize: fast, moderately fast and slow. To obtain a rapid therapeutic effect, rapid stepwise saturation with medilazide is used for 24–36 h (rapid saturation type). In this case, the initial dose is 0.8 - 1.0 mg, followed by the addition of 0.1 - 0.15 mg every 5 - 6 hours to obtain the optimal therapeutic effect. After that, switch to maintenance doses (0.1 - 0.4 mg per day).
For moderately rapid digitalization, the drug is prescribed in the first - second - the third day at a dose of 0.6 - 0.8 mg (0.2 mg 3 - 4 times a day), on the fourth, fifth and sixth - 0.4 - 0 , 6 mg (0.2 mg 2 to 3 times a day).
Then, they switch over to maintenance doses - from 0.1 to 0.4 mg (on average, 0.2 mg per day).
For slow digitalization, medilazide is given for 10 days at a dose of 0.4–0.6 mg (0.2 mg 2–3 times a day), subsequently switching to maintenance doses (0.1–0.2 mg per day ).
The highest daily dose is 1, 25 mg.

Product form: tablets of 0.0001 g (0.1 mg) in a package (test tubes) of 50 pieces.

The foreign drug bomecor corresponding to medilazide is also available as a solution in ampoules of 2 ml each (0.2 mg of β-methyldigoxin), and in the form of drops for oral administration, containing 0.6 mg of b-methyldigoxin in 1 ml (45 drops). Ampoules are used if necessary, rapid saturation.
