Description of the medicine: Pipothiazine (Pipotiazine)
PIPOTIAZINE (Pipotiazine). 2-Dimethylsulfamido-10- [3- (4-hydroxyethylpiperidino) propyl] phenothiazine.
Synonyms: Piportil, Piportil, Pipothiazine.
In chemical structure, it is close to thioproperazine, differs from it by the presence of an oxyethylpiperidinyl propyl group in the side chain at the nitrogen atom instead of methylpiperazinyl propyl.
It has a pronounced and prolonged antipsychotic effect; reduces psychomotor agitation, psychotic fear, aggressiveness.
It is used for the paranoid form of schizophrenia, hebephrenia, chronic psychoses with hallucinations, chronic delirium, as well as for manic seizures, arousal states, and other mental disorders.
Assign orally in the form of tablets or drops for chronic psychoses of 20-30 mg per day in one dose, after stabilization - 10-20 mg per day (outpatient). In acute cases, it is prescribed orally for several days at 30-60 mg per day, then 10-20 mg per day.
After stabilization, the drug can be administered intramuscularly in the form of a special dosage form << Piportil L 4 >> (piportil palmitate);
Piportil L 4 is a solution of piportil palmitate (2.5%) in oil. The drug acts for a long time, as the active substance is released during hydrolysis after injection. The solution is administered intramuscularly 1 time in 4 weeks. Intravenous administration is not allowed. The average dose for an adult is 100 mg (4 ml of 2.5% solution). An elderly patient with epilepsy, suffering from alcoholism, mentally unbalanced is initially given a lower dose (25 mg), then the dose is gradually increased.
Intramuscular administration of piportil L 4 is allowed only after checking the tolerability of the drug (pipothiazine) when taken orally.
Possible side effects and precautions are the same as when treated with other phenothiazine antipsychotics. In case of an overdose, acute parkinsonism syndrome and coma are possible.
The drug is contraindicated in agranulocytosis, porphyria, angle-closure glaucoma; urinary retention may occur with urethroprostatic disorders. In patients with epilepsy, a decrease in the seizure threshold is possible.
Method of release: pipothiazine (piportil) is available in yellow tablets of 10 mg (in a package of 200 pieces); in the form of a 4% solution in vials with 10 ml pipettes (400 mg in a vial). One drop of a water-alcohol solution corresponds to 1 mg of the active substance; piportil L 4 - in the form of a 2.5% solution in oil in ampoules of 4 and 1 ml (100 and 25 mg in an ampoule). For injection use only glass syringes.
